Özet

Bu çalışmada delici olmayan yaralanma sonucu oluşan göz küresi yırtığı ve konjonktiva altına kristalin lensin yer değişimi gelişen 70 ya- şında bir kadın olgu sunulmuş ve cerrahi tedavisinin tartışılması amaçlanmıştır. Olgunun darbe gelen gözünde görme keskinliği ışık hissi se- viyesinde olup, üst nazal konjonktiva altı alanda kabarıklık ve o bölgede sarımsı yansıma görüntüsü izlendi. Göz ileri derecede hipoton oldu- ğu için skleral yırtık ve lens yer değişimi şüphesiyle hasta ameliyata alındı. Ameliyat esnasında kristalin lensin tamamen skleral yırtık bölge- sinden konjonktiva altına yer değiştirdiği saptandı. Kristalin lens temizlenip, birincil yırtık onarımı uygulandı. İlk ameliyattan 2 ay sonra vit- reus içi kalıcı hemoraji nedeniyle pars plana vitrektomi uygulandı. Düzeltilmiş görme keskinliği 2/10 seviyesine kadar yükseldi. Delici olma- yan göz yaralanması nedeniyle acile başvuran hastalarda konjonktiva altında kütle görüntüsü var ise lenste yer değişimi mutlaka düşünülme- li ve acil cerrahi tedavi uygulanmalıdır.

Abstract

In this study, a 70 year-old female with a rarely seen subconjunctival cristallin lens dislocation after blunt ocular trauma was presented and its surgical treatment was discussed. The visual acuity of the traumatic eye was light perception and superonasal subconjunctival eleva- tion and yellowish reflex at that region was observed. The patient was operated with the suspicion of scleral rupture and lens dislocation due to the hypotonic state of the eye. During the operation it was noticed that the crystalline lens was dislocated completely from the scleral rup- ture area to the subconjunctiva. The crystalline lens was removed and primary perforation repair was performed. Two months after the first surgery, the patient had pars plana vitrectomy, because of the persistent vitreous hemorrhage. Best-corrected visual acuity increased upto 2/10. If there is a subconjunctival elevation in a patients presenting to the emergency with blunt eye trauma, lens dislocation should be considered and urgent surgical treatment should be performed.
