Özet
Amaç: Göz kapağı birincil kötü huylu tümörlerinin klinik, histopatolojik özellikleri ve tedavi sonuçlarını değerlendirmek. Gereç ve Yöntem: Göz kapaklarında kötü huylu tümör nedeni ile ameliyat edilen 38 hasta; yaş, cinsiyet, yerleşim, klinik görünüm, oluşu- mun boyutu, histopatoloji, cerrahi teknik ve nüks yönünden geriye dönük olarak incelendi. Tüm hastalara tam oftalmolojik muayene, kapak ve orbita incelemesi yapılarak, orbita tutulumu şüphesi olan olgularda bilgisayarlı tomografi / manyetik rezonans görüntüleme ile yayılım dere- cesi belirlendi. Bulgular: Otuz sekiz hastanın yaş ortalaması 70,2 (38-95) yıl idi. Histopatolojik dağılım; bazal hücreli karsinom: 27 (%71,1), yassı hüc- reli karsinom: 6 (%15,8), sebase bez karsinomu: 3 (%7,9) ve malign melanom: 2 olgu (%5,3) şeklinde idi. Ortalama tümör çapı 19,58 mm (min: 4, max: 50) idi. On dört (%37) olguda daha önce geçirilmiş cerrahi öyküsü ve nüks tümör mevcuttu. Toplam 5 olguya egzenterasyon uygulan- dı. Takipte toplam üç olguda (%7) nüks oluştu; ortalama tekrarlama süresi 20 ay (3-63 ay arası) idi. Nükseden oluşumların üçü de iç kantus yerleşimli olup, kliniğimize nüks tümör olarak başvurmuş olan ilerlemiş olgulardı. Bunlardan biri, dondurulmuş kesit denetimi ile cerrahi sı- nırlar temiz olarak bildirilmiş olmasına rağmen nükseden bir olgu idi. Sonuç: Bazal hücreli karsinom ve yassı hücreli karsinom göz kapaklarında en sık görülen kötü huylu tümörlerdir. Günümüzde cerrahi te- davi hala en güvenli ve başarılı seçenek olmaya devam etmektedir. Nükslerin önlenebilmesi için özellikle kantus yerleşimi veya daha önce nüks etmiş olması gibi yüksek risk taşıyan olgularda, tümörden arınmış cerrahi sınırlar önemlidir. Cerrahi sınır kontrolü ve donmuş kesit denetimi son derece faydalı olmasına rağmen yine de nüks görülebileceği için hastaların bir süre izlenmesi yararlı olacaktır.
Abstract
Objective: To evaluate the clinical and histopathological characteristics and treatment results of malignant eyelid tumors. Material and Method: Thirty eight patients who underwent malignant eyelid tumor surgery were evaluated retrospectively for age, sex, clinical features, lesion size and localization, histopathology, surgical technique and recurrences. All patients underwent full ophthalmologic examination including eyelid and orbita. Computer tomography or magnetic resonance imaging was used for evaluation of orbital involvement. Results: The mean age was 70.2 (38-95)years. The histopathological examination revealed basal cell carcinoma in 27 (71.1%), squamous cell carcinoma in 6 (15.8%), sebaseous gland carcinoma in 3 (7.9%) and malignant melanoma in 2 (5.3%) patients. The mean tumor size was 19.58 mm (min: 4, max: 50). Fourteen patients (37%) previously had recurrent tumor and history of eyelid tumor surgery. Five patients un- derwent exenteration. Recurrences were observed in 3 (7%) case at the mean time of 20 months (3-36 months). All these 3 cases were advanced; previously recurrent cases and lesions were located in the medial canthal area. One of these had recurred even though the surgical margins were reported as clean with frozen section examination. Conclusion: Basal cell carcinoma and squamous cell carcinoma were the most frequent malignant tumors of the eyelid. Today, surgical treatment remains the most successful and safest alternative. Tumor-free surgical margins are important for the prevention of recurrences, es- pecially in high-risk cases such as recurrent or canthal tumors. Recurrences can be seen although tumor free surgical margins and frozen sec- tion examination are very useful, thus patients’ should be followed up for a time.
