Duane Retraksiyon Sendromlu Olgularda Refraktif Özellikler, Ambliyopi Oranları ve Üç Boyutlu Görme Fonksiyonunun Değerlendirilmesi

Özet

Amaç: Duane retraksiyon sendromu, değişik miktarlarda addüksiyon kısıtlılığı ile beraber abdüksiyon kısıtlanması, addüksiyonda kapak aralığının daralması, glob retraksiyonu ve yukarı/aşağı atımın eşlik edebildiği konjenital bir sendromdur. Son sınıflamalarda Duane retraksiyon sendromu, konjenital kraniyal disinervasyon hastalıkları içinde anılmaya başlanmıştır. Bu çalışmada, Duane retraksiyon sendromlu hastalarda refraktif değerler, Duane retraksiyon sendromu alt tiplerinin ambliyopi oranları ve üç boyutlu görme fonksiyonunun değerlendirilmesi amaç- lanmıştır. Gereç ve Yöntem: Bu retrospektif çalışmada, Ocak 1995 ile Aralık 2016 tarihleri arasında takip ve tedavisi yapılan Duane retraksiyon sen- dromulu 632 hasta incelendi. En az 6 ay takibi olan hastalar, çalışmaya dahil edildi. Tüm hastalara rutin oftalmolojik muayene, ortoptik de- ğerlendirme ve %1 siklopentolat hidroklorür damla ile siklopleji uygulandı. Çalışmada Duane retraksiyon sendromu hastalarındaki refraktif de- ğerler, Duane retraksiyon sendromu alt tiplerine göre ambliyopi oranları ve üç boyutlu görme fonksiyonları değerlendirildi. Bulgular: Duane retraksiyon sendromlu 632 hastanın yaş ortalaması 11,3± 9,4 yaş (en az 6 ay- en fazla 57 yaş) idi. 337 hastada (tüm has- taların %53,3’ünde), düşük derecede hipermetropi (+1,50 ile +4,50 diyoptri arası) mevcuttu. Düşük derecede hipermetropi, çalışma grubu- muzda en yaygın görülen sferik refraktif değer idi. Tüm hastaların sferik refraktif değerlerinin ortalaması, +1,89 diyoptri hipermetropi olarak saptandı. Ambliyopi 632 hastanın 148’inde (%23,4), kliniğe ilk başvurduğu anda mevcuttu. Kapama tedavisi sonrası yapılan en son muayene- lerde, 75 hastada (%11,8) ambliyopi mevcuttu. Ambliyopi oranları açısından tip I, II ve III Duane Sendromlu hastalar arasında istatistiksel ola- rak anlamlı fark saptanmadı (p=0,18). Stereopsisi değerlendirilebilen 528 hastanın 412’sinde (%78) stereopsis mevcut idi. Tip I, II ve III Duane retraksiyon sendromu olgularında stereopsisi olan hastaların oranı açısından istatistiksel olarak anlamlı fark saptanmadı (p=0,14). Sonuç: Duane retraksiyon sendromu, kırma kusurlarından genellikle hipermetropinin baskın olduğu, ambliyopi oranlarının normal top- lumdan yüksek olduğu, stereopsisin bozulmasının çok beklenmediği konjenital bir sendromdur. Hastalara ayrıntılı bir oküler muayene, ortoptik değerlendirme ve stereopsis muayenesi yapılması, hastalığın tedavi ve takibi için kritiktir.

Abstract

Objective: Duane retraction syndrome is a congenital syndrome that is characterized by adduction and/or abduction limitation in eye move- ments, narrowing of palpebral fissure in adduction, with/without glob retraction and up/downshoot. In the recent classifications, Duane Syndrome was included in the congenital cranial dysinnervation disorders. In this study, we aimed to evaluate refractive features, amblyopia rates and stereopsis in Duane retraction syndrome cases. Material and Method: 632 patients with duane retraction syndrome who were followed and treated from January 1995 to December 2016 were evaluated in this retrospective study. The patients followed at least six months were included in the study. Routine ophthalmic examination and orthoptic assessment were performed for all patients. Cycloplegia with 1% cyclopentolate HCL eye drops were applied in all patients. Refractive features in Duane retraction syndrome patients, amblyopia rates and stereopsis of Duane retraction syndrome subtypes were analyzed. Results: The mean age of 632 patients with Duane retraction syndrome was 11.3 ± 9.4 years (min: 6 months-max: 57 years). 337 of 632 pa- tients (%53.3) had moderate hypermetropia (hypermetropia between +1.50 and +4.50 diopters). Moderate hypermetropia was the most com- mon refractive error. The average of spherical refractive error values in all patients was +1.89 diopter hypermetropia. 148 of 632 patients (%23.4) had amblyopia at the time of admission. After occlusion therapy for treatment, amblyopia was present in 75 patients (%11,8) in the last examinations. There was no statistically significant difference in the rates of amblyopia between patients with type I, II and III Duane Syndrome (p=0.18). Stereopsis could be evaluated in 528 patients. 412 of 528 patients (%78) had stereopsis. No statistically significant difference was found between patients with duane retraction syndrome type I, II and III in terms of presence of stereopsis (p= 0.14). Conclusion: Duane retraction syndrome is a congenital syndrome in which hypermetropia is the dominant refractive error, amblyopia rates are higher than normal population and stereopsis is not expected to deteriorate substantially. A detailed ocular examination, orthoptic assess- ment and stereopsis examination are critical for treatment and follow-up of the disease.
