Herpetik Ön Üveitlerde Klinik Bulgular ve Tedavi Sonuçları

Özet
Amaç: Herpetik ön üveitlerin tanısında klinik bulguların önemini vurgulamak ve tedavi sonuçlarını değerlendirmek. Gereç ve Yöntem: 2007-2018 yılları arasında kliniğimizin üvea biriminde herpetik ön üveit tanısıyla takip edilen 21 hastanın 26 gözüne ait kayıtlar retrospektif olarak incelendi. Bulgular: Çalışmaya dahil edilen 21 hastanın, yaş ortalaması 55,7 (13-76) yıl, ortalama takip süresi 35,7 (3-132) aydı. Olgularımızda akut dönemde değişen derecelerde ön kamara reaksiyonu (eser/+4 hücre) mevcuttu. On dokuz gözde (%73,1) ön üveit kornea tutulumu ile birlikte idi ve bunların 15’inde kornea skarı (%57,7), 4’ünde epitel defekti (%15,4) mevcuttu. Sekiz gözde (%30,8) yama veya sektöryel iris atrofisi, 11 göz- de (%42,3) transillüminasyon defekti, 7 gözde (%26,9) pupil distorsiyonu, 9 gözde (%34,6) arka sineşi mevcuttu. Olguların en iyi düzeltilmiş görme keskinliği Snellen eşeline göre ortalama 0,56’dan tedavi sonrası 0,66’a arttı. Ortalama göz içi basıncı 17,5 (13-22) mmHg saptandı an- cak 14 göz (%53,9) topikal antiglokomatöz tedavi altındaydı. Takiplerde 16 gözde (%61,5) nüks gözlendi. Sonuç: Herpetik ön üveit tanısı klinik bulgularla konulabilir. Tek taraflı üveite eşlik eden sektöryel iris atrofisi, transillüminasyon defekti, pupil distorsiyonu ve akut göz içi basıncı yüksekliği durumunda ayırıcı tanıda herpetik ön üveit düşünülmelidir. Uzun süreli oral asiklovir teda- visi komplikasyonları önlemede ve nüks oranlarını azaltmada etkilidir.
Abstract
Objective: Of the study is to emphasize the importance of clinical findings in diagnosis and to evaluate the treatment results. Material and Method: The records of 26 eyes of 21 patients who were followed with the diagnosis of herpetic anterior uveitis in the uvea unit of our clinic between 2007-2018 were retrospectively analyzed. Results: Of the 21 patients included, the mean age was 55.7 (13-76) years, and the mean follow-up period was 35.7 (3-132) months. All of the eyes had anterior chamber reaction (trace / + 4 cells). Nineteen eyes (73.1%) had anterior uveitis with corneal involvement and 15 of them had corneal scar (57.7%) and 4 had epithelial defects (15.4%). Eight eyes (30.8%) had sectorial iris atrophy, 11 eyes (42.3%) had transillumi- nation defect, 7 eyes (26.9%) had pupillary distortion and 9 eyes (34.6%) had posterior synechia. The best-corrected visual acuity of the cases increased from 0.56 to 0.66 after treatment. The mean intraocular pressure was 17.5 (13-22) mmHg, but 14 eyes (53.9%) were under topical antiglaucomatous therapy. Recurrence occurred in 16 eyes (61.5%). Conclusion: Diagnosis of herpetic anterior uveitis can be done by clinical findings. In case of unilateral uveitis associated with sectoral iris atrophy, transillumination defect, pupillary distortion and acute intraocular pressure elevation, herpetic anterior uveitis should be considered in the differential diagnosis. Long-term oral acyclovir treatment is effective in preventing complications and reducing recurrence rates.
