Diyabetik Retinopati Komplikasyonlarında 23-G ve 25-G+ Transkonjonktival Vitrektominin Kısa Dönem Sonuçlarının Karşılaştırılması

Özet

Amaç: Diyabetik retinopati komplikasyonlarının tedavisinde yüksek hızlı 23-G ve 25-G+ transkonjonktival vitrektominin kısa dönem sonuçlarını karşılaştırmaktır. Gereç ve Yöntem: Diyabetik retinopati komplikasyonları için primer pars plana vitrektomi uygulanan 120 hastanın 125 gözü geriye dönük olarak incelendi. Cerrahiler 23-G ve 25-G+ sistemi kullanılarak Alcon Constellation cihazı ile gerçekleştirildi. 23-G ve 25-G+ vitrektomi ameliyatları, intra-operatif ve ilk 2 hafta içinde görülen erken post-operatif komplikasyonlar, intraoküler tamponad gereksinimi, sklerotomi sütürü konması gereksinimi ve bimanuel cerrahi oranları açısından geriye dönük olarak değerlendirildi ve karşılaştırıldı. Bimanuel cerrahi oranlarının karşılaştırılması yalnız 23-G veya 25-G+ sistemlerinin kullanıldığı dönemler için yapıldı. Bulgular: 23-G grubunda 43, 25-G+ grubunda ise 82 göz vardı. İki grup arasında sklerotomi sütür gereksinimi, intra-operatif ve erken post-operatif komplikasyonlar açısından anlamlı fark bulunmadı. İki grup arasında intraoküler tamponad cinsinin dağılımı açısından fark yoktu. Bimanuel cerrahi yalnız 23-G ve yalnız 25-G+ kullanılan dönemlerde karşılaştırıldığında 23-G grubunda olguların %30’unda, 25-G+ grubunda ise %6,5’inde gerekli idi (p=0.012). Sonuç: Diyabetik retinopati komplikasyonlarının tedavisinde 23-G ve 25-G+ yüksek hızlı vitrektomi eşit etkili ve güvenlidir. Bimanuel cerrahi gereksinimi, 25-G+ vitrektomi ile 23-G ile vitrektomiye göre daha düşüktür

Abstract

Objective: To compare high speed 23-G and 25-G+ transconjunctival vitrectomy surgery for complicatons of diabetic retinopathy in short term. Material and Method: One hundred twenty five eyes of 120 patients who had primary pars plana vitrectomy for complications of diabetic retinopathy were retrospectively analyzed, and comparesion was made between 23-G and 25-G+ surgeries for intra-operative and early post-operative complications such as intraocular tamponade, sclerotomy suture requirement rates and bimanuel surgery rates seen during the post-operative 2 weeks retrospectively. Bimanuel surgery rates were compared for the periods in which only 23-G or 25-G+ systems were used. Results: There were 43 eyes in the 23-G and 82 eyes in the 25-G+ group. There was no significant difference in rates of sclerotomy suture requirement, intra-operative and early post-operative complications between 23-G and 25-G+ groups. The distribution of types of tamponade was not different between 23 G and 25-G+ surgeries. Bimanuel surgery was required in 30% and 6.5% of 23-G and 25-G+ when compared for periods in which only 23-G or 25-G+ were used (p=0.012). Conclusion: Both 23-G and 25-G+ high speed vitrectomy are equally safe and effective in short term in the management of complications of diabetic retinopathy. Bimanuel surgery requirement is less with 25-G+ compared to 23-G surgery.
