Özet

Amaç: Pediatrik katarakt cerrahisi sonrası şaşılık ve ambliyopi gelişimini araştırmak. Gereç ve Yöntem: Pediatrik katarakt sebebiyle ameliyat olan hastaların dosyaları incelendi. Ameliyat tipi, görme keskinliği, fiksasyon tercihi, kayma miktarı, füzyon ve stereopsis bakıldı. Kayması olan hastaların fiksasyon süreleri değerlendirildi. Her iki gözde fiksasyon 5 saniyenin üzerinde ise görme eşit, 5 saniyenin altında ve/veya gözü kırpma ile fiksasyon sağlanıyorsa “hafif ambliyopi”, fiksasyon yoksa “derin ambliyopi” olduğu kabul edildi. Bulgular: Bir ay ile 8.5 yaş arasında, 40’ı kız, 47’si erkek, 87 hastanın 147 gözü incelendi. Altmış hasta bilateral, 27 hasta tek taraflı, 48 hasta 1 yaş altı, 39 hasta 1 yaş üstü ameliyat edildi. Yüz sekiz göze limbal fakoemülsifikasyon, arka kapsüloreksis, ön vitrektomi, 21 göze limbal fakoemülsifikasyon, arka kapsüloreksis, ön vitrektomi, arka kamara göz içi merceği ve 18 göze ise limbal fakoemülsifikasyon, arka kamara göz içi merceği uygulandı. 42 hastada kayma olup içe kayma (%29,9), dışa kaymaya (%18,4) göre daha sıktı. Alternasyonu olan 17 hastanın 1’inde hafif ambliyopi saptandı. Alternasyonu olmayan 25 hastanın 20’sinde ambliyopi saptandı. Ambliyopi, tek taraflı kataraktlı hastalarda daha fazlaydı (p=0,001). Tek taraflı cerrahi geçiren 4, bilateral cerrahi geçiren 19 hastada füzyon mevcuttu (p=0,126). Stereopsis bilateral cerrahi geçiren 3 hastada mevcuttu. 20’si tek taraflı, 23’ü bilateral cerrahi geçiren 43 hastaya kapama uygulandı. Kapamaya uyumu iyi olan %11,8, kapamaya uyumu kötü olan %33,3 hastada görme keskinliği 0,1’in altında idi (p=0,141). Nistagmus 29 hastada gözlendi. Sonuç: Bilateral kataraktı olan erken cerrahi geçirmiş hastalarda görme keskinliği ve stereopsisin daha iyi olduğu gözlendi. Pediatrik kataraktta erken tanı ve tedavinin görsel gelişim için önemli olduğunu düşünmekteyiz.

Abstract

Objective: To investigate the development of strabismus and amblyopia after surgery for pediatric cataract. Material and Method: The records of patients who underwent cataract surgery for pediatric cataract were investigated. Type of surgery, visual acuity, fixation preference and presence of stereopsis were noted. Fixation times of patients with deviation were evaluated. Visual acuity of both eyes were accepted as equal when the fixation times were 5 seconds or more in each eye. When the fixation time was under 5 seconds or blink was needed for fixation, the diagnosis was accepted as mild amblyopia. Profound amblyopia was accepted when there was no fixation in deviated eye. Results: The age range of 40 female, 47 male patients (147 eyes) was between one month and 8,5 years. The surgery was performed bilateral in 60, unilateral in 27 patients and in the first year of life in 48 patients. Limbal phacoemulsification, posterior capsulorhexis, anterior vitrectomy were performed in 108 eyes, limbal phacoemulsification, posterior capsulorhexis, anterior vitrectomy and posterior chamber intraocular lens implantation were made in 21 eyes, limbal phacoemulsification and posterior chamber intraocular lens implantation were made in 18 eyes. Deviation developed in 42 patients, esotropia was more often than exodeviation (29.9%, 18.4%,respectively). Mild amblyopia was detected in one eye of 17 patients with no fixation preference. Profound amblyopia was detected in 20 of 25 patients without alternation and amblyopia was more often in patients with unilateral cataract (p=0.001). The motor fusion was present in 4 patients who underwent unilateral and in 19 patients who underwent bilateral surgery (p=0.126). Stereopsis was detected in 3 patients who underwent bilateral surgery. The patch treatment was carried out in 43 patients including 20 patients with previous unilateral and 23 patients with bilateral congenital cataract. The visual acuity was under 0.1 in 11.8% of patients with good compliance and in 33.3% of patients with poor compliance (p= 0.141). Nystagmus was detected in 29 patients. Conclusion: Patients who underwent earlier surgery for bilateral cataract were found to have better visual acuity and stereopsis. Earlier diagnosis and management seems to be important for better visual development in pediatric cataract.
