Kırk Yaş Üstü Keratokonus Hastalarında Hastalığın İlerleme Analizi

Progression Analysis in Patients with Keratoconus Over 40 Years

Özet

Amaç: Kırk yaş üstü hiçbir tedavi almamış keratokonus hastalarında keratokonusun ilerlemesini değerlendirmek. Gereç ve Yöntem: Bu retrospektif, klinik çalışmaya 40 yaş üstünde keratokonus tanısı almış fakat daha önce hiçbir tedavi uygulanmamış ve en az 1 yıllık takibi olan hastalar dahil edildi. Hastaların en iyi düzeltilmiş görme keskinliği, manifest sferik ve silindirik refraksiyon değerleri, sferik eşdeğeri ve topografi verileri karşılaştırıldı. Maksimum keratometri (Kmaks) değerlerine göre ilerleme, gerileme ve stabilizasyon oran- ları incelendi. Bulgular: Çalışmaya 31 hastanın 61 gözü dahil edildi. Hastaların %48,3’ü kadın %51,6’sı erkekti. Hastaların ilk geliş yaşı ortalama 44,2±4,9 yıl, son geliş yaşı ortalama 47,6±5,5 yıl olup, ortalama takip süresi 35,4 ± 14,6 ay idi. Hastaların ilk geliş ve son geliş en iyi düzeltil- miş görme keskinliği sırasıyla 0,10±0,18 ve 0,12±0,18 logMAR olup, ortalama fark 0,01±0,12 idi (p=0,49). Ortalama manifest sferik ve silin- dirik değerler ve ortalama keratometri (Kort), K1, K2 değerlerinde istatistiksel olarak anlamlı değişim izlenmedi (sırasıyla p=0,87, p=0,23, p=0,98, p=0,77, p=0,84). İlk geliş Kmaks 51,91±4,80, son geliş Kmaks 51,64±4,85 olup, ortalama fark -0,26 ±0,86 idi (p=0,02). Dokuz hasta- da gerileme izlenirken diğer hastalar stabil seyretti ve hiçbir hastada ilerlemeye rastlanmadı. Ortalama santral kornea kalınlığı, en ince kornea kalınlığı, ön elevasyon, arka elevasyon, korneal hacimleri, topografik silindirik ve sferik eşdeğerleri ilk geliş ve son geliş muayeneleri arasında istatistiksel olarak anlamlı fark gözlenmedi (sırasıyla p = 0,61, p = 0,78, p = 0,19, p = 0,26, p = 0,47, p = 0,53, p = 0,53). Sonuç: Kırk yaş üzeri tanısı konulmuş keratokonus hastalarında takip süresi boyunca ilerleme görülmemiştir. Bazı hastalar hatta Kmaks de- ğerlerindeki spontan azalma nedeniyle bir miktar gerileme gösterebilmektedir. Diğer topografik ve refraktif parametreler ve görme keskinliği stabil kalmıştır.

Abstract

Objective: To evaluate the progression of keratoconus in keratoconus patients over 40 years old, who did not received any treatment. Material and Method: Patients who were diagnosed with keratoconus over the age of 40 but had not received any treatment before and had at least 1-year follow-up period were included in this retrospective, clinical study. The best corrected visual acuity, manifest, spherical and cylin- drical refractive values, spherical equivalent and topographical data were compared. Progression, regression, and stabilization rates according to the maximum keratometry (Kmax) values were examined. Results: Sixty-one eyes of 31 patients were included in this study. Of these patients, 48.3% were female and 51.6% were male. The mean age at first visit was 44.2 ± 4.9 years and the mean age at last visit was 47.6 ± 5.5 years and the mean follow-up period was 35.4 ± 14.6 months. The mean best corrected visual acuity levels at the first and last visits were 0.10 ± 0.18 and 0,12 ± 0.18 logMAR, respectively, and the mean dif- ference was 0.01 ± 0.12 (p=0,49). There were no statistically significant changes in the mean manifest spherical and cylinder values, mean K1, K2 and mean K (p =0.87, p=0.23, p=0.98, p=0.77, and p=0.84, respectively). The first visit Kmax was 51.91 ± 4.80 D and the last visit Kmax was 51.64 ± 4.85 D. A mean difference of -0.26 ± 0.86 D was observed (p=0.02). Nine patients had regression while the others were stable or did not show any progression. The mean central corneal thickness, the thinnest corneal thickness, anterior elevation, posterior elevation, corneal volumes, and spherical equivalent value were not significantly different between the first and the last visits (p=0.61, p=0.78, p=0.19, p=0.26, p=0.47, p=0.53, and p=0.53, respectively). Conclusion: No progression was seen in keratoconus patients diagnosed over the age of 40 during the follow-up period. Some of the pa- tients may even show regression due to the spontaneous decrease in their Kmax values. Other topographical and refractive parameters, and vi- sual acuity level remained stable.
