Özet

Amaç: Medikal tedaviye dirençli kornea erimesine yol açan kornea ülser olgularında 360 derece konjonktiva tenon kompleksi ile uygulanan örtme tedavisinin sonuçlarını değerlendirmek. Gereç ve Yöntem: Ocak 2018 ve Temmuz 2019 tarihleri arasında kliniklerimize başvuran spontan kornea perforasyonu veya desmatosel sebebi ile konjonktiva tenon kompleksi örtme tedavisi uygulanmış olan hastaların dosyaları retrospektif olarak incelendi. Hastaların demografik özellikleri, takip süreleri, anatomik başarı oranları ve flep retraksiyonu zamanları kaydedildi. Bulgular: Çalışmaya 9 hastanın 9 gözü dahil edildi (8 erkek, 1 kadın, ortalama yaş: 53± 15,4 yıl). Tüm hastaların takip süreleri en az 6 aydı (ortalama 12,3 ±2,1 ay). Kornea erimesine yol açan patolojiler; nörotrofik keratit (n=5), bakteriyel keratit (n=3) ve kimyasal yanığa sekonder limbal kök hücre yetmezliği (n=1) olarak saptandı. Bütün olgularda perforasyon/desmatosel alanının 2 ay içerisinde sikatrizasyon ile iyileştiği ve konjonktivanın limbus gerisine retrakte olduğu izlendi. Anatomik başarı %100 olarak kaydedildi. Sonuç: Tedaviye dirençli kornea erimesine yol açan ağır kornea ülserleri tedavisinde örtücü konjoktiva tenon kompleksi flepleri; ülserin daha kısa sürede, daha düşük maliyetle ve daha yüksek hasta konforu ile kapanmasını sağlayarak, keratoplasti için uygun alıcı yatak oluşturulmasını sağlamaktadır.

Abstract

Objective: To evaluate the results of total conjunctival covering applied with 360 degree conjunctival tenon complex in corneal ulcer cases that are resistant to medical treatment. Material and Method: The files of the patients who underwent conjunctiva-tenon complex flap surgery for the management of spontaneous corneal perforation or desmatocele were retrospectively reviewed. Demographic features of the patients, follow-up times, anatomic success rate and flap retraction times were further recorded. Results: Nine eyes of 9 patients were included (8 males, 1 female, mean age 53± 15.4 years). The follow-up times of the patients was at least 6 months (mean: 12.3 ±2.1 months). The disorders that lead to corneal melting were: neurotrophic keratitis (n=5), bacterial keratitis (n=3) and limbal stem cell deficiency secondary to chemical burn (n=1). In all cases, the perforation/desmatocele area healed through cicatrization within two months and conjunctiva-tenon complex retracted beyond limbus to reveal the healed but partiallyopaque corneal tissue. Anatomical success rate was recorded as 100%. Conclusion: In the management of resistant corneal ulcers that lead to corneal melting with or without spontaneous corneal perforation, conjunctiva-tenon complex flaps helped the healing of the resistant corneal ulcer to heal in a shorter time, with reduced cost, with relatively high patient comfort, and supported the formation of a suitable recipient bed for future keratoplasty.
