Acil Olarak Penetran Keratoplasti Uygulanan Hastaların Klinik Bulguları ve Görsel Sonuçları

Özet
Amaç: Bu çalışmada kliniğimizde acil olarak penetran keratoplasti uygulanan hastaların endikasyonları, klinik bulguları ve görsel sonuçlarını değerlendirilmek. Gereç ve Yöntem: Retrospektif olarak yaptığımız bu çalışmaya Aralık 2016-Mayıs 2018 tarihleri arasında kliniğimizde acil olarak penetran keratoplasti uygulanan olgular dahil edildi. Hastaların endikasyonları, klinik bulguları, görsel sonuçları, greft sağkalım oranları, göz korunma oranları, intraoperatif ve postoperatif komplikasyonları değerlendirildi. Bulgular: 20 hastanın 23 gözüne acil penetran keratoplasti uygulandı. Hastaların 12’si erkek, 8’i kadındı. Yaş ortalaması 61,8±16,6 (28- 90) yıldı. Tüm hastaların preoperatif görme keskinliği ≤ 0,008 idi. Ortalama takip süresi 6±4,8 (2-18) aydı. En sık acil penetran keratoplasti endikasyonu keratite bağlı perforasyondu. Cerrahi sonrası iki göze (%8,6) tekrar acil penetran keratoplasti uygulandı. Fitizis bulbi 4 gözde (%17,3) ve grefon yetmezliği 3 gözde (%13,0) gelişti. 15 gözde (%65,2) greft saydamlığı elde edildi. Ameliyat sonrası görülen komplikasyonlar sütürlerde gevşeme (n=9 göz), persistan epitel defekti (n=6 göz), grefonda keratit (n=3 göz), koroid dekolmanı ve hipotoni (n=1 göz), retina dekolmanı (n=2 göz), endoftalmi (n=1 göz) idi. Cerrahi sonrasında gözlerin %43,4’ünde vizyon artışı elde edildi. Sonuç: Acil penetran keratoplastide greftin saydamlığının sürdürmesi diğer penetran keratoplasti endikasyonlarına göre daha düşük olmakla beraber dikkatli bir ameliyat sonrası, altta yatan hastalığın sebebine yönelik ek tedaviler ve yakın takiple görsel anlamda iyi neticeler elde etmek mümkündür.
Abstract
Objective: The aim of this study was to evaluate the indications, clinical findings and visual outcomes of patients who underwent urgent penetrating keratoplasty in our clinic. Material and Method: Patients who underwent emergency penetrating keratoplasty in our clinic between December 2016 and May 2018 were included in this retrospective study. Indications, clinical findings, visual outcomes, graft survival rates, glob salvage rates, intraoperative and postoperative complications were evaluated. Results: Twenty-three eyes of 20 patients underwent emergency penetrating keratoplasty. Twelve patients were male and 8 were female. The mean age was 61.8±16.6 (28-90) years. Preoperative visual acuity of all patients was ≤ 0.008. The mean follow-up period was 6 ± 4.8 (2-18) months. The most common indication for emergency penetrating keratoplasty was perforation due to keratitis. Two patients (8.6%) underwent repeat penetrating keratoplasty after surgery. Ftisis bulbi developed in 4 eyes (17.3%) and graft failure in 3 eyes (13.0%). Graft transparency was achieved in 15 eyes (65.2%). Postoperative complications include loosening of sutures (n=9 eyes), persistent epithelial defect (n=6 eyes), graft keratitis (n=3 eyes), choroidal detachment and hypotonia (n=1 eye), retinal detachment (n=2 eyes), endophthalmitis (n=1 eye). After surgery, vision increase was achieved in 43.4% of eyes. Conclusion: While maintaining the transparency of the graft in emergency penetrating keratoplasty is lower than the other indications of penetrating keratoplasty, it is possible to obtain good visual results after careful operation, additional treatments for the cause of the underlying disease and close follow-up.
