Korneal İncelme ve Delinmelerde Tektonik Lamellar Kornea Yama Greftinin Etkisi

Özet

Amaç: Korneal incelmesi olan hastalarda yapılan tektonik lamellar kornea yama greftinin etkinliğini incelemek. Gereç ve Yöntem: Ocak 2015-Temmuz 2019 tarihleri arasında kornea incelme veya perforasyonuna bağlı lamellar kornea yama grefti uygulanan hastaların kayıtları retrospektif olarak incelendi. Endikasyonlar, anatomik başarı, görme keskinliği ve komplikasyonlar değerlendirildi. Bulgular: Çalışmada 21 hastanın 21 gözü incelendi. On dört erkek (%72,7), 7 kadın hasta (% 27,2) ortalama 17,6±14,7 ay (3-35 ay) takip edildi. Etyolojiye bakıldığında; sekiz olguda sistemik otoimmün hastalıklara bağlı oküler yüzey bozukluğu, beş olguda enfeksiyöz keratit, üç olguda travma sekeli, iki olguda oküler skatrisyel pemfigoid, bir olguda trahom sekeli, bir olguda Mooren ülseri, bir olguda keratokonusa bağlı erime mevcuttu. Hastalardan 16’sına kornea yama grefti öncesi amniyotik membran transplantasyonu uygulandı. Ameliyattan önce 16 gözde korneal perforasyon mevcuttu ve beş göz perforasyon riski taşımaktaydı. Hastaların ameliyat öncesi düzeltilmiş uzak görme keskinlikleri ortalama 1,84±1,37 logMAR iken ameliyat sonrası düzeltilmiş görme keskinlikleri 1,22±1,26 logMAR düzeyinde idi. Ameliyat sonrasında tüm gözlerde yeterli kornea kalınlığı elde edildi. Bir hastada üç ay sonra greft üzerinde herpes simpleks epitelyal keratiti, bir hastada bir ay sonra greft epitelinde açıklık, bir hastada interfaz keratiti izlendi. Sonuç: Tektonik lamellar kornea yama grefti uygulaması, kornea incelmesi ve perforasyonu olan olgularda glob bütünlüğünü yeniden oluşturan ve oküler yüzey stabilizasyonu sağlayan etkili bir cerrahi yöntemdir. Ayrıca tektonik lamellar kornea yama grefti uygulaması ile göz bütünlüğü ile birlikte kabul edilebilir bir görme düzeyi elde etmek de mümkündür.

Abstract

Objective: To investigate the efficacy of tectonic lamellar corneal patch graft in patients with corneal thinning. Material and Method: The records of patients who underwent lamellar corneal patch grafting due to corneal thinning or perforation between January 2015 and July 2019 were reviewed retrospectively. Indications, anatomical success, visual acuity and complications were evaluated. Results: In this study, 21 eyes of 21 patients were included. Fourteen males (72.7%) and 7 females (27.2%) were followed for an average of 17.6±14.7 months (3-35 months). When the etiology is examined; eight patients had ocular surface disorder due to systemic autoimmune diseases, five had infectious keratitis, three had traumatic sequelae, two had ocular scatricial pemphigoid, one had trachoma sequelae, one had Mooren’s ulcer and one had keratoconus. Sixteen patients underwent amniotic membrane transplantation prior to corneal patch grafting. Corneal perforation was present in 16 eyes before the operation and five eyes were at risk of perforation. The mean preoperative corrected visual acuity of the patients was 1.84±1.37 logMAR and the postoperative corrected visual acuity was 1.22±1.26 logMAR. Adequate corneal thickness was obtained in all eyes postoperatively. One patient had herpes simplex epithelial keratitis three months later, one patient had graft epithelium and one patient had interphase keratitis. Conclusion: Tectonic lamellar corneal patch graft is an effective surgical method that regenerates globe integrity and provides ocular surface stabilization in patients with corneal thinning and perforation. It is also possible to achieve an acceptable level of vision with tectonic lamellar corneal patch graft.
