Tiroid Oftalmopatili Olgularda Koroid Kalınlığının Değerlendirilmesi ve Sağlıklı Gözlerle Karşılaştırılması

Özet

Amaç: Tiroid oftalmopatinin koroid kalınlığı üzerine olan olası etkilerini spektral optik koherans tomografi bulguları aracılığı ile değerlendirmek. Gereç ve Yöntem: Ocak 2016-Ocak 2018 tarihleri arasında başvuran tiroid oftalmopati olduğu belirlenen 30 hastanın 60 gözü ve herhangi bir oküler hastalık veya cerrahi öyküsü olmayan 30 sağlıklı bireyin 60 gözü (kontrol grubu) bu vaka-kontrol çalışmasına dahil edildi. Koroid kalınlığı subfoveal alan ve foveanın 1000 μm nazal ve 1000 μm temporalinden olmak üzere toplam üç bölgeden spektral optik koherans tomografinin genişletilmiş derinlik görüntüleme tekniği ile ölçüldü. Yapılan değerlendirmeler sağlıklı gözlerle karşılaştırıldı. Bulgular: Tiroid oftalmopati grubunda yaş ortalaması 40,6±13,4 yıl (19-64 yıl) iken, grup 2’de 38,4±10,1 yıldı (26-58 yıl). Ortalama subfoveal, nazal ve temporal koroid kalınlıkları tiroid oftalmopati grubunda sırasıyla 332,4±85,3 μm (166-566 μm), 305,6±89,4 μm (147- 539 μm) ve 315,9±86,8 μm (163-531 μm) iken, kontrol grubunda bu ölçümler sırasıyla 397,4±96,2 μm (189-643 μm), 350,6±99 μm (160- 622 μm) ve 377,6±97,7 μm (179-715 μm) idi. Tüm lokalizasyonlarda koroidal kalınlıkları, tiroid oftalmopati grubunda kontrol grubuyla kıyaslandığında istatistiksel olarak ince bulundu (p≤0,01). Sonuç: Tiroid oftalmopati olgularının koroid kalınlığının kontrol grubuna göre anlamlı ölçüde ince olduğu tespit edilmiştir. Bu incelme tiroid oftalmopatinin patofizyolojisi açısından önem teşkil edebilir.

Abstract

Objective: To investigate the possible effects of thyroid ophthalmopathy on the choroidal thickness measured by spectral domain-optical coherence tomography. Material and Method: Sixty eyes from 30 patients with thyroid ophthalmopathy (21 men, 9 women) and 60 eyes from 30 healthy individuals (20 men, 10 women) without any ocular disease or surgery history who applied between January 2016 and January 2018 were recruited for this case-control study. Choroidal thickness measurements were obtained using enhanced depth imaging with spectral domain- optical coherence tomography from 3 regions including subfovea, 1000 μm nasal and 1000 μm temporal regions of fovea. Assessments were compared with the healthy eyes’ measurents. Results: The mean age was 40.6±13.4 (19-64) years in the thyroid ophthalmopathy group and 38.4±10.1 (26-58) years in the control group. The mean subfoveal, nasal and temporal choroidal thickness in the thyroid ophthalmopathy group were 332.4±85.3 μm (166-566 μm), 305.6±89.4 μm (147-539 μm) and 315.9±86.8 μm (163-531 μm) respectively. On the other hand, the control group measurents were 397.4±96.2 μm (189-643 μm), 350.6±99 μm (160-622 μm) and 377.6±97.7 μm (179-715 μm) respectively. There was significant difference in choroidal thickness in all areas between the groups and choroidal thickness measurents were lower in the thyroid ophthalmopathy group (p≤0.01). Conclusion: Choroidal thickness of the patients with thyroid ophthalmopathy were significantly lower than the healthy individuals. This thinness can be important for the pathophysiology of the thyroid ophthalmopathy.
