Kronik Retina Dekolmanlı Olgu Serisinde Klinik Özellikler, Tedavi ve Takip Sonuçları

Özet

Amaç: Kronik veya uzun süreli retina dekolmanı olgularının klinik özellikleri, tedavi endikasyonları ve cerrahi uygulanan olguların te-davi sonuçlarının değerlendirilmesi. Gereç ve Yöntem: Çalışmaya kronik retina dekolmanı tanısı konan 21 hastanın 21 gözü dahil edildi. Klinik olarak takip edilen veya cer-rahi uygulanan vakalar retrospektif olarak görsel prognoz, anatomik başarı ve retinada meydana gelen değişiklikler açısından incelendi. Bulgular: Olguların ortalama takip süresi 6-50 ay (ortalama 16,5 ay) idi. Demarkasyon hattı olan 10 olgudan makülası yatışık olan 3 olgu ve maküladan demarkasyon hattı geçen 2 olgu sadece klinik izlemle takip edilirken, 3 olguda maküla yatışık iken; eşlik eden sub-retinal sıvı varlığı nedeni ile demarkasyon hattının çevresine bariyer lazer uygulandı. 2 olguya 6 aydan uzun süreli maküla dekolmanı varlığına bağlı düşük görsel beklenti nedeni ile operasyon planlanmadı. 11 olguda demarkasyon hattı olup olmaması önemsenmeden anatomik ve fonksiyonel başarı beklentisiyle 6 olgu subretinal bant, 5 olgu maküla dekolmanı nedeniyle cerrahi endikasyon aldı. Tüm popülasyonda görme keskinlik seviyeleri 12 olguda arttı, 6 olguda değişmedi, 3 olguda azaldı. Sonuç: Kronik retina dekolmanı olgularında makülanın tutulumu, subretinal bantların varlığı, demarkasyon hattına yakın subretinal sı-vı görülmesi anatomik ve/veya fonksiyonel başarıyı etkileyen önemli prognostik faktörlerdir. Bu parametrelere dikkat edilerek klinik ta-kip ve tedavinin yapıldığı olgularda, tekrar eden lazer ve cerrahi işlemler gerekse de görme keskinliğinde artış sağlanabilmektedir.

Abstract

Objective: Clinical features of chronic or long-term retinal detachment cases, treatment indications and evaluation of treatment results of patients undergoing surgery. Material and Method: 21 eyes of 21 patients diagnosed with chronic retinal detachment were included in the study. Clinically monitored or surgically treated cases were examined retrospectively for visual prognosis, anatomical success, and changes in the retina. Results: The mean follow-up period of the cases was 6-50 months (mean 16.5 months). Of the 10 cases with demarcation line; 3 cases with attached macula and 2 cases with demarcation line crossing through the macula were only followed up by clinical monitoring, while in 3 cases barrier laser was applied all around the demarcation line due to the presence of subretinal fluid although the macula was at-tached. The surgical approach was not planned in 2 cases due to the low visual acuity expectation because of the presence of macular detachment for more than 6 months. 6 eyes with subretinal band and 5 eyes with macular detachment totally 11 eyes were operated with the expectation of anatomical or functional recovery regardless of whether there is a demarcation line or not. In all the population vi-sual acuity was improved in 12 cases, was stabil in 6 cases and decreased in 3 cases. Conclusion: Macular involvement, the presence of subretinal bands, and the presence of subretinal fluid around the demarcation line are important prognostic factors affecting anatomical and/or functional success in patients with chronic retinal detachment. In cases where the clinical follow-up and treatment were performed with paying attention to these parameters, a recovery in visual acuity can be achieved although it is required repeated laser and surgical procedures.
