Özet

Amaç: Bu çalışmanın amacı, tiroid oftalmopatili hastalarda üst kapak retraksiyonunun te- davisi amacıyla subkonjunktival yolla enjekte edilen Botulinum Toksin Tip A’nın (BTA) etkin- liğini ve yan etkilerini değerlendirmektir. Gereç ve Yöntem: Çalışmamızda distiroid oftalmopatiye bağlı üst kapak retraksiyonu tes- pit edilen 27 olgunun 33 gözü, aynı şartlarda çekilmiş fotograflarda incelenerek, retrospektif olarak değerlendirildi. Ortalama yaşı 41±10,9 (25-63) olan 16 kadın, 11 erkek olgunun tümü ötiroid durumda idi. Olguların oftalmolojik muayeneleri yapıldı ve kapak-refle uzaklığı uygu- lama öncesinde ölçüldü. Doz saptanmasında retraksiyon miktarı dikkate alındı. Doz ikiye bö- lünerek, üst göz kapağı tarsının üst sınırında, medial ve temporal kenarlara subkonjunktival yolla 2-15 IU BTA enjeksiyonu yapıldı. Olgularda kenar-refle uzaklığı (KRU) enjeksiyon son- rası 7-10. günde tekrar ölçüldü. Yetersiz düzelme olanlara 2-4 IU ek BTA enjekte edildi. Hastalar 4-6 ay boyunca 2 aylık aralıklarla takip edilerek KRU değerleri ölçüldü. Ayrıca ağ- rı, hematom, diplopi, pitoz, orbiküler yetmezlik gibi komplikasyon ve yan etkiler not edildi. Bulgular: BTA enjeksiyonu sonrası 33 gözün 23’ünde hedeflenen kenar-refle uzaklığı olan 3-4 mm seviyesine ulaşıldı (%69,6). Sekiz gözde (%24), enjeksiyon sonrası kapak retraksiyo- nu % 50’sinden daha az düzeldiğinden onuncu günde ek BTA enjeksiyonu uygulandı. Üç göz- de pitoz gözlenirken, 1 gözde kenar- refle uzaklığında 0,5 mm artış saptandı. Olguların kenar- refle uzaklıkları 4-6 ay içinde eski değerlerine yaklaştı. Tüm yan etkiler 4 ay içinde sekelsiz dü- zeldi. Sonuç: Tiroid oftalmopatili olgulardaki üst kapak retraksiyonunun tedavisinde subkon- junktival yolla BTA enjeksiyonu, kapak cerrahisine kadar geçen dönemde etkisi ve yan etkile- ri geçici, etkin, güvenilir ve tekrarlanabilir bir tedavi yöntemidir.

Abstract

Aim: The aim of this study is to evaluate the efficacy and complications of subconjunctival Botulinum toxin type- A (BTA) injection, for the management of upper eyelid retraction asso- ciated with dysthyroid ophthalmopathy. Material and Method: In this study, 33 eyes of 27 patients with upper eyelid retraction due to dysthyroid ophthalmopathy, were reviewed retrospectively according to the photographs taken under standardized conditions. Sixteen female and 11 male patients with mean age 41 ±10.9 (25-63) were in the euthyroid state. Patients had complete ophthalmic examinations and margin-reflex distances were measured before injection. BTA was injected subconjunctivally, just above the superior tarsal border of the upper eyelid in doses between 2-15 IU, depending on the measured retraction amount. 7-10 days after injection margin- reflex distances were measured. Two to four IU BTA injections were repeated in patients who were undercorrected. All patients were followed for margin reflex distance measurements regularly for 4 to 6 months. All complications, such as pain, diplopia, ptosis, and orbicularis insufficiency were also recorded. Results: After Botulinum Toxin-A injections, targeted margin-reflex distance, which is 3-4 mm, was reached in the 23 of 33 eyes (69.6%). 8 eyes (24%) who had remaining lid retraction more than 50% of preinjection retraction amount were re-injected again on the tenth day. 0.5 mm increase in retraction was seen only in one eye Ptosis was observed in 3 eyes. Upper eye- lid retraction, close to the preinjection measurements, recurred after 4-6 months in all patients. Side effects resolved spontaneously in 4 months without sequelae. Conclusion: In dysthyroid ophthalmopathy, subconjunctival injection of Botulinum toksin A is an effective, safe, transient, and repeatable method with few complications during the waiting period before lid surgery.
