Özet

Amaç: Merkezimizde son 5 yıl içerisinde implante edilen kardiyak ritim düzenleyici cihazların çeşitlerini, işlem özelliklerini ve tekniklerini, işlem sırasında-sonrasında oluşan komplikasyonları belirlemektir. Gereç ve Yöntem: 1 Ocak 2014 ve 31 Ocak 2019 tarihlerini kapsayacak şekilde retrospektif bir tarama yapıldı. Verilerine ulaşılabilen hastaların klinik özellikleri, cihaz uygulama endikasyonları, işlem özellikleri, işlem sırasında ve sonrasında yaşanan komplikasyonları değerlendirildi. Bulgular: 375 kardiyak implante edilebilir cihaz işlemi hastasının kaydına ulaşıldı. Hastaların ortanca yaşları 70 (60-78) iken, %63,7’si ise erkek idi. Hastaların %43.2’sine kalp pili implantasyonu yapılırken (n=162), %45’ine implante edilebilir kardiyoverter defibrilatör ve kardiyak resenkronizasyon terapisi-defibrilatör cihazı implantasyonu yapılmıştır(n=169). İşlemlerin %11,8’ini ise cihaz jeneratör replasmanları oluşturmakta idi (n=44). Kalp pili endikasyonları, AV bloklar (%72,8) ve sinüs nodal problemler ve hasta sinüs sendromu (%27,2) idi. İmplante edilebilir kardiyoverter defibrilatör ve kardiyak resenkronizasyon terapisi-defibrilatör endikasyonları sırası ile iskemik kardiyomiyopati (%65,08), dilate kardiyomiyopati (%29,58), hipertrofik kardiyomiyopati (%4,14), uzun QT sendromu (%0,6) ve Brugada send romu (%0,6) idi. İşlemlerin tüm kısa ve uzun dönemli komplikasyon oranı %8,8 idi. En sık görülen komplikasyonlar sırası ile pil cebi ve lead revizyonu, kanama ve hematom, pnömotoraks, pil cebi enfeksiyonu, enfektif endokardit ve perikardiyal efüzyon idi. Sonuç: Merkezimizde kalp pilleri ve diğer implante edilebilen kardiyak cihazlar başarı ile ve güncel literatürle benzer komplikasyon oranları ile implante edilmektedir.

Abstract

Objective: The aim of this study was to determine the types of cardiac rhythm devices implanted in last 5 years, the characteristics of the procedure, the techniques used in the procedure, the complications of the procedure and the infections associated with the device implantation. Material and Method: A retrospective screening was conducted including the dates January 1, 2014 and January 31, 2019. The clinical features of the patients whose data were available, the indications of the device, the characteristics of the procedure and the complications during and after the procedure were evaluated. Results: A record of 375 patients was reached. The median age of the patients was 70 (60-78), and 63.7% of them were male. While implantation of pacemaker was performed in 43.2% of the patients, implantable cardioverter defibrillator and cardiac resynchronization therapy-defibrillator device implantation were performed in 45% (n = 169). 11.8% of all procedures were device generator replacements (n=44). The indications for pacemaker implantation were AV blocks (72,8%) and sinus nodal problems and the sick sinus syndrome (27,2%). The indications for implantable cardioverter defibrillator and cardiac resynchronization therapy-defibrillator were respectively ischemic cardiomyopathy (65.08%), dilated cardiomyopathy (29.58%), hypertrophic cardiomyopathy (4.14%), long QT syndrome (0.6%) and Brugada syndrome (0.6%). The overall complication rate was 8.8%. The most common complications were the need for battery pocket and lead revision, hemorrhage and hematoma, pneumothorax, battery pocket infection, and infective endocarditis and pericardial effusion. Conclusion: In our center, pacemakers and other implantable cardiac devices are implanted successfully and with similar complication rates as the current literature.
