Özet
Endodontik tedavi sonrası, aşırı madde kaybına uğramış dişlerin restorasyonları, klinik açıdan birtakım güçlükler doğurmaktadır. Yapılacak restorasyonun çeşidi mevcut diş kronunun miktarına göre belirlenmelidir. Koronal desteğini büyük ölçüde kaybetmiş kanal tedavili dişlerin restorasyonunda genellikle döküm post-core veya fabrikasyon post destekli protetik kronlar tercih edilmektedir. Bununla beraber, son yıllarda yeni ad-heziv dental materyal ve post tekniklerinin geliştirilmesinden dolayı, günümüzde kanal tedavili dişlerin konservatif uygulamalarla tedavisi tercih edilmektedir. Bu tür konservatif tedavilerde döküm posta alternatif olarak, fabrikasyon post destekli veneering amalgam-komposit resin restorasyon tekniklerinin kullanımı önerilmektedir. Bu olgu bildiriminde bir erkek hastanın, travma sebebiyle fraktüre uğramış ve geniş çürük nedeniyle aşın madde kaybı gösteren, sağ üst ikinci premolar dişine, kanal tedavisi sonrası fabrikasyon post destekli kombine amalgam-kompozit veneer restorasyonu uygulanmıştır. Bu olgu sunumunun yapılma amacı; kronal desteğini büyük ölçüde kaybetmiş endodontik tedavili premolar bir dişte, fabrikasyon post destekli amalgam core-kompozit veneer kombine restorasyon tekniğinin klinik uygulama açısından uygunluğunu tartışmaktır.
Abstract
The restoration of severely damaged teeth lead to some clinical problems after the endodontic treatment. The amount of the remained tooth structure identifies the choice of treatment which is going to be performed. Post-core and/or dowel retained casting restorations are usually recommended in the restoration of the teeth that had undergone extensive coronal damage. However due to the recent developments in adhesive technology and post insertion techniques, conservative manner is usually preferred to treat the endodontically treated teeth. As a conservative approach, it is recommended to use a dowel retained amalgam-composite veneer restoration which is an alternative to casting posts. In this case, a combined post-retained amalgam-composite veneer restoration is applied to the maxillary right second premolar of a male patient which is severely damaged due to trauma and extensive dentin caries, after endodontic treatment. The aim of this case report is to discuss whether the insertion of the fabricated post retained amalgam-composite veneer restoration technique performed in endodontically treated premolars, is being applicable in routine clinical procedure.
