Özet

Restoratif amaçlı diş kesimi sonucu dentin tübüllerinin açığa çıkması, bakteri invazyonu, pulpal reaksiyon ve aşırı has¬sasiyet riskini beraberinde getirir. Açığa çıkan dentin tübüller-ine bağlı olarak bakteri ve bakteri ürünleri tübüllere infiltre olabilir ve pulpal reaksiyonlara yol açabilir. İlaveten, dentin tübülleri içindeki sıvının hareketi ağrıya neden olabilir. Bu sonuçlara bağlı olarak dentin tübüllerinin hemen kesim son¬rası kapatılması pulpanın canlılığının korunması için önem¬lidir. Geçici restorasyonlar dişin termal uyaranlar ve mikrosızıntıya karşı korunmasını sağlar. Bununla beraber geçici restorasyon¬ların ve simanların mikrosızıntının engellenmesinde tamamı ile etkili olamadığı gösterilmiştir. "Çift bonding tekniği" açığa çıkmış dentin yüzeylerine diş preparasyonundan hemen sonra bonding uygulaması şeklindedir. Bu yaklaşımın temel yararı dentin tübüllerinin preparasyon sonrası kapatılarak, bakteriyel infiltrasyon ve hassasiyetin engellenmesidir.

Abstract

During the tooth preparation for a restoration dentin tubules are exposed and this creates a potential risk of bacterial invasion, pulpal reactions and hypersensitivity. During this period bacteria and bacterial products can infiltrate the tubules and create adverse pulpal reactions and hydraulic fluid flow within dentin tubules may create hypersensitivity. Related to these consequences it seems to be crucial to seal the dentin tubules right after the tooth preparation in order to protect pulp vitality. Provisional restorations provide protection for the tooth from the negative effects of thermal stimuli and microleakage. However, it has been shown that provisional restorations and luting agents do not provide complete sealing of the restorations. "Dual-bonding technique" requires hybridization of exposed dentin surface right after the tooth preparation. The main advantage of this approach is sealing of the dentin tubules immediately after tooth preparation to prevent bacterial infiltration and sensitivity.
