Özet: Acarbose, mikrobial orjinli (actinoplanes) bir pseudotetrasakkarittir ve ince barsak fırçamsı kenar enzimleri olan glukoamilaz, sukraz, maltaz, dextrinaz ve pankreatik a-amilaz aktivitesini inhibe eder, bu yüzden komplex karbonhidratlardan glukoz emilimini geciktirir. Bu çalışmada Acarbose'un 38 Tip II diabet hastasında karbonhidrat ve lipid metabolizması üzerine etkileri incelendi. 15 gün, günde üç defa 50 mg. p. o verilen acarbose dozu günde üç defa 100 mg. p. o ile 10 hafta daha devam edildi. Hastalarda Acarbose 'un açlık ve tokluk 1. ve 2. saat kan glukoz düzeyleriyle TG, LDL, VLDL ve total kolesterol düzeylerinde düşmeye neden olduğu görüldü. HDL kolesterol düzeylerindeki yükselme istatistiksel olarak anlamlıydı.Diabet regülasyonundaki düzelmeyi Hemoglobin A 1c düzeylerindeki anlamlı düşüş desteklemekteydi. 38 hastanın 9'unda (%23.7) gastrointestinal yan etkiler görülmekle birlikte, bunlar tedavinin sonlandırılmasını gerektirmemiştir. Sonuç olarak Acarbose'un Tip II diabet hastalarında hem açlık hem post-prandial hiperglisemi durumlarında tercih edilebilecek, kan lipid profili üzerinde de olumlu sonuçları olan bir tedavi yöntemi olduğuna kanaat getirilmiştir.

Abstract

Summary: Acarbose is pseudotetrasaccharide with a micrabial origin (actinoplanes) and it causes a delay in the absorbtion of glucose from complex carbohydrates via inhibition of small intestinal brush border enzymes such as glucoamilase, sucrase, maltose, dextrinase and pancreatic a-amilase. We studied the effects of acarbose or, carbohydrate and lipid metabolism in 38 Type II diabetic patients. Acarbose was given per orally 50 mg. three times a day for 15 days and later on it was given per orally 100 mg. three times a day for 10 weeks more. It was shown that acarbose caused decrement in fasting and post-prandial 1. and 2. hour blood glucose levels and also in TG, LDL, VLDL and total cholesterol levels. The increase in HDL cholesterol level was statistically significant. The decrement in hemoglobin A<sub>1c</sub> levels with acarbose therapy supported the better diabetes regulation. In 9 of 38 patients (23.7%) gastrointestinal side effects were seen, but these didn't require the cessation of therapy. We concluded that acarbose is a drug which can be preferred for both fasting and post-prandial hyperglycemia and has positive effects on blood lipia profile in Type H diabetic patients.
