Oral liken planus patogenezinde mast hücresi ve adezyon moleküllerinin rolü ve önemi

Özet
Bu çalışmanın amacı oral liken planus (OLP) patogenezinde mast hücresi ve adezyon moleküllerinin rolünü araştırmaktır. Çalışmaya 40 oral liken planus (OLP), 20 dermal liken planus (OLP) ve 12 non-spesifık inflamatuar fibröz. hiperplazi (NSİFH) dahil edilmiştir. Tüm parafın kesitler mast hücre boyası (Unno) ile ve immünhistokimyasal olarak ICAM-1 ve VCAM-1 için avidin-biyotin-peroksidaz yöntemi ile boyandı. Mast hücre yoğunluğu lokalizasyon ve lezyon farkı olmaksızın yakın bulgulandı. Buna karşılık ICAM-1 ve VCAM-1 ekspresyonu arasında farklar izlendi. Bu da mast hücresinin likenoid infıltrattaki rolünün özgün olmadığını düşündürdü. ICAM-1 ve VCAM-1 hücresel ekspresyonda farklılık göstermezken vasküler ICAM-1 VCAM-1 'den daha yüksek bulgulandı. Bu bulgular ICAM-1 ve VCAM-1 'in belirli bir lezyonun patogenezinde farklı hücresel ve vasküler ekspresyon paternleri ile rol aldıklarını düşündürdü. Hücresel ekspresyonda benzer davranan ICAM-1 ve VCAM-1 vasküler endotelde farklı donanımlar gösterdi. Normalde zayıf eksprese olan VCAM-1'in likenoid lezyonların submukozasında NSİPH'deki kadar ve daha yüksek eksprese olması VCAM-1'in bu lezyonlarda özgün bir rolü olduğunu düşündürdü. Yüksek garede 'U ekspresyon sonuçları, toplam ekspresyon sonuçlarıyla uyumlu bulgulandı. Bu da dansitenin fonksiyonla ilintili olarak değişebildiğim ve değerlendirmelerde gözönünde tutulması gerektiğini düşündürdü.
Abstract
In the study, 40 oral lichen planus (OLP), 20 dermal lichen planus (DLP) and 12 non specific inflammatuar fibros hiperplasi (NSIFH) existing the archivel materials of the Pathology Department have been testhed. All formalin fixed, paraffin embedded sections have stained by the mast cell stain, Unno, and by using the technic of immunohistochemical staining with the avidin-biotin-peroxidase method for ICAM-1 and VCAM-1. The assessment has been done by the five-point grading system by examining the vascular (perivascular for the mast cell) and cellular (scattered for the mast cell) structures of the subepithelial and submucosal areas. The intensity of the mast cells were similar in three lesional groups and in both location (subepithelial and submucosal). On the other hand, the ICAM-1 and VCAM-1 expression showed differences between the groups. These findings suggested us that the role of the mast cell in the lichenoid infiltrate was not unique and there might be some other inductive factors. While ICAM-1 and VCAM-1 did not show difference on the cellular expression, ICAM-1 was identified higher than VCAM-1 on the vascular expression. These results made us think that ICAM-1 and VCAM-1 have played a role through the different cellular and vascular expression patterns in the pathogenesis of a defined lesion.
