Özet

Bu çalışma, midazolamla sağlanan sedasyondan sonra flumazenilin etkilerini değerlendirmek için yapılmıştır. SSK Ankara Eğitim Hastanesinde ürolojik cerrahi müdahale planlanan, ASA sınıflandırmasına göre L, 2. ve 3. grupta yer alan 40 hasta bu çift kör çalışmaya dahil edildi. Epidural anesteziyi takiben 2-4 mg midazolam i. v. bolus olarak verildi. Antagonist ajan verilmesi için olgular rastgele iki gruba ayrıldı. Gruplara ya flumazenil ya da plasebo verildi. Tüm hastalarda sistolik ve diastolik kan basıncı, kalp hızı, solunum sayısı, sedasyon skoru, kısa kognitif muayene skoru, resimli kart testi skoru, modifiye steward skala skoru ve retrograd amnezi skoru ve ilaç yan etkileri kaydedildi. Sonuç olarak, epidural anestezi esnasında kullanılan midazolamın sedatif amnestik, oryantasyon, adaptasyon, kooperasyon, motor aktivite üzerine olan etkileri flumazenil ile kısa sürede geri çevrilebilir.

Abstract

This study was performed to evaluate the effects of flumazenil after sedation with midazolam. Fourty, ASA class I, II, and III, patients undergoing urological surgery in S.S.K. Ankara Hospital were included in this double-blind study. Following epidural anaesthesia, midazolam (2-4 mg, iv.) was given to the patients as a bolus dose. Patients were, randomly divided into two groups for application of the antagonist agent. Each group was administered either flumazenil (Group 1) or placebo (Group 2). All of the patients were monitorized for the sedation score, both systolic and diastolic blood pressures, hearth rate, respiration rate, the score of short cognitive examination, the score of picture identification test, the score of modified steward scale, the retrograde amnesia score, and also drug-adverse effects. As a result, we concluded that flumazenil can reverse the effects of midazolam under epidural anaesthesia such as amnesia, orientation, adaptation, co-operation, motor activation in a short period.
