Hiperlipidemi osteoporoz ilişkisi

Özet
Amaç: Hiperlipidemi ile osteoporoz arasındaki ilişki yeterince aydınlatılmamıştır. Çalışmamızda osteoporoz ile hiperlipidemi arasında ilişki olup olmadığını belirlemeyi amaçladık. Gereç ve Yöntemler: Femoral ve lomber bölgeye yönelik dual-energy x-ray absorbtiometry (DEXA) yöntemi uygulanarak kemik mineral dansitometri skoru belirlenen 100 olgu çalışmaya dahil edildi. KMD ölçümü sonucunda herhangi bir bölgeye ait T skoru -1.0 olanlar normal kabul edildi. Öncesinde lipit düşürücü veya osteoporoza yönelik tedavi alan hastalar ve sekonder bir nedene bağlı osteoporozu bulunan vakalar çalışma dışı bırakıldı. Hastaların DEXA yöntemi ile ölçülen femoral ve lomber bölge T skorları, lipit düzeyleri (total kolesterol, HDL, LDL, trigliserit) değerleri kaydedildi. Bulgular: Çalışmaya yaş ortalaması 61.2 (SD:± 8.9) olan 90 kadın olgu ile yaş ortalaması 63.4 (SD:± 15.7) olan 10 erkek olgu dahil edildi. 48 olguda (%48) osteoporoz, 36 olguda osteopeni (%36) saptanırken, 16 (%16) olgu ise normal olarak değerlendirildi. Osteoporozu olanlarda total kolesterol ve LDL düzeyleri sırasıyla 216 mg/dl ve 128 mg/dl, osteopeniklerde 194 mg/dl ve 111 mg/dl ve KMD değerleri normal olan olgularda 180 mg/dl ve 100 mg/dl olarak saptandı ve osteoporozu olanlarla KMD'si normal olanlar arasında total kolesterol ve LDL düzeyleri açısından istatistiksel olarak anlamlı fark bulundu (sırasıyla p=0.03 ve p=0.01). Hiperkolesterolemisi olanlarda femoral bölge toplam T skoru ve lomber bölge toplam T skoru sırasıyla -0.92 ve -1.89 olarak belirlendi. Kolesterol düzeyleri normal olanlarda ise sırasıyla -0.26 ve -0.92 idi. Kolesterol düzeyi normal olanlarla hiperkolesterolemisi olanlar arasında femoral bölge toplam T skoru ve lomber bölge toplam T skorunun karşılaştırılması sonucunda istatistiksel olarak anlamlı fark saptandı (p=0.01 ve p=0.001). Osteoporozu olanlarda trigliserit ve HDL değerleri sırasıyla 185 mg/dl ve 45mg/dl, osteopenisi olanlarda 171 mg/dl ve 42 mg/dl ve KMD değerleri normal olanlarda ise 158 mg/dl ve 43 mg/dl olarak saptandı. Gruplar arasında trigliserit ve HDL açısından fark yoktu. Sonuç: Total kolesterol ve LDL düzeyleri ile osteoporoz gelişimi arasında anlamlı ilişki mevcuttur. Hiperlipidemi saptanmış hastalarda osteoporoz açısından da inceleme yapılması uygun olur.
Abstract
Objective: The relation between osteoporosis (OP) and hyperlipidemia (HL) has not been fully clarified. We aimed to determine if there is a relation in between osteoporosis and hyperlipidemia. Material and Methods: Patients of whom bone mineral density (BMD) scores that have been defined by dual energy x-ray absorptiometry (DEXA) belonging to femoral and lumbar region, have been included in the study. As a result of BMD measurement patients who had T score <-2.5 was accepted as osteoporotic, second group who had T score between -1.0 and -2.5 was accepted as osteopenic, third group who had T score >-1 was accepted as normal. Patients who have been under the treatment of OP and who had used lipid reducers were eliminated from the study. In addition to this, patients who have OP secondary to another reason were eliminated, too. Lipid (total cholesterol, HDL, LDL, triglyceride) levels were also recorded. Results: Average total cholesterol and LDL levels were respectively 216 mg/dl and 128 mg/dl in patients with OP, 194 mg/dl and 111 mg/dl in patients with osteopenia and 180 mg/dl and 100 mg/dl in normal people and statistically significant difference has been found regarding total cholesterol levels and LDL levels in between patients with OP and patients with normal BMD (respectively p=0.03 and p=0.01). Femoral region total T scores (FTT) of patients with hypercholesterolemia was -0.92 and lumbar region total T scores(LTT) of patients with hypercholesterolemia was -1.89. T scores of patients with normal cholesterol levels were respectively -0.26 and -0.92. As a result when FTT and LTT were compared, in patients with normal level of cholesterol and with hypercholesterolemia, statistically significant difference has been found (respectively p=0.01 and p=0.001). Conclusion: There is a significant relation in between total cholesterol and LDL levels, and osteoporosis development. It can be useful to do research about osteoporosis on patients with hyperlipidemia.
