Kronik otitis medialı hastalarda mastoid cerrahisi sonrası fasial sinirin değerlendirilmesi

Özet
KBB hekimleri tarafından sıkça uygulanan bir prosedür olan mastoid cerrahisi sonrası fasial sinir fonksiyonlarının ne ölçüde etkilendiğini ortaya koyan bir yayın literatürde mevcut değildir. Böyle bir cerrahi sonrası fasial sinirin ne ölçüde etkilendiğini Elektronörografi (ENoG)'yi kullanarak kantitatif olarak saptamayı amaçladık. Kronik Otitis Media (KOM) tanısıyla cerrahi işlem uygulanan ve takibi yapılan 32 hasta üzerinde uygulandı. Anamnezinde fasial paralizi, kulak operasyonu, menenjit ve kafa travması bulunan hastalar çalışmaya alınmadı. ENoG'yi uygularken testler arası değişkenliği (TAD) en aza indirgemek için optimize teknikle alar kayıt yöntemini kullanmayı uygun gördük. Her hastaya, preoperatif bir kez (1. ölçüm), postoperatif iki kez (2. ve 3. ölçüm) olmak üzere toplam üç kez ENoG uygulandı. Preoperatif ve postoperatif dönemde elde edilen fasial sinire ait amplitüd ve latans değerleri karşılaştırıldı. Mastoid cerrahisi uygulanan KOM'lu hastaların preoperatif ve postoperatif dönemlerde fasial fonksiyonları ENoG kullanılarak kıyaslandığında istatistiksel olarak anlamlı bir fark bulunmamıştır (p>0.05). Ancak özellikle radikal cerrahiler uygulanan hastalarda 1. ile 2. ölçüm arasında amplitüd değerlerinde %37' lere varan uç değerlerin elde edilmesi dikkat çekiciydi. Mastoid cerrahinin fasial sinir işlevleri üzerine etkisinin olmadığı sonucuna istatistiksel olarak varmış olsak da, özellikle radikal cerrahiler sonrası erken dönemde elde ettiğimiz uç değerlerin olması fasial sinirin cerrahi işlemden etkilendiğinin kanıtıdır. ENoG'nin sinir hasarını değerlendirmek için zaman sınırlamasının olması cerrahi işlemin geç dönem etkilerini değerlendirmede yetersiz kalmaktadır.
Abstract
Mastoid surgery is performed frequently by otorhinolaryngologists and yet there are not any articles concerning the status of facial nerve after the surgery. We aimed to determine the impact of surgery on facial nerve by the use of electroneurography (ENoG). This study is conducted on 32 patients with chronic otitis media (COM) in whom mastoid surgery had been performed and had been taken into the follow up program. Patients who had facial palsy, mastoid surgery, menegitis and head trauma were not included in the study. In order to minimize the intertest differences, we used alar recording tecnique for ENoG. ENoG was per-formed preoperatively (1st measurment) once and postoperatively twice (2nd and 3rd measurements). The latency and amplitude values are compared. Facial nerve function detected by ENoG before and after mastoid surgery did not differ statistically (p>0.05). However it was noteworthy to detect extreme values (measurements) up to 37% in amplitudes between the first and the second measurements for the radical procedures. Although we conclude statistically that mastoid surgery does not have any impact on facial nerve func-tion the extreme values obtained just after the radical procedures indicate an interaction. ENoG, a practical method of evaluating the facial nerve implies that facial nerve function is not damaged by the mastoid surgical procedures.
