Özet

Açlık sırasında kulakta uğultu, çınlama ve geçici işitme kaybı tarif eden üç hastaya uzamış oral glikoz tolerans testi ve eş zamanlı olarak açlık ve glikoz yüklemesi sonrası saatlik saf ses odyometrisi ve "Transient Evoked Otoacoustic Emission" (TEOAE) bakılmıştır. Reaktif hipoglisemi tanısı alan bu hastaların plazma glikoz değerlerinin düşmesi ile saf ses eşiklerinde düşme olduğu gözlenmiştir. Eş zamanlı bakılan otoakustik emisyon testlerinde TEOAE amplitüdlerinde oluşan değişiklikler bu durumun sübjektif bir faktöre bağlı olmadığını düşündürmüştür. Glikoz değerlerinin yükselmesi ile bu değişiklikler geri dönmektedir. Bu üç hastadan elde edilen bulgular plazma glikoz düzeyinin kohlea fonksiyonu ve özellikle dış tüy hücreleri üzerinde etkili olduğunu düşündürmektedir.

Abstract

Three patients complaining about tinnitus and temporary hearing loss were investigated with extended oral glucose tolerance test, simultaneous pure tone audiometry and transient evoked otoacoustic emission. These patients were diagnosed with reactive hypoglycemia and their results showed pure tone threshold shifts simultaneos with drop in blood sugar levels. Simultaneous decrements in TEOAE amplitudes suggested that these changes were not relat-ed to a subjective factor. After elevation of blood glucose level, shifts in audiologic findings were reversed. Our findings in these three patients suggested that blood glucose level is an important factor on cochlear function.
