Özet

Hidransefali, serebral hemisferlerin tamamına yakınının olmaması ile karakterize nadir bir santral sinir sistemi malformasyonudur. Anne baba arasında akrabalık olmayan, 20 yaşındaki annenin 2. gebeliğinin 38. haftasında ultrasonografi ile semilobar holoprozensefali ön tanısı konmuş; yine aynı haftada 2600 gr olarak doğan bebekte solda anoftalmi, sağda mikroftalmi, basık burun kökü, düzleşmiş filtrum ve mikrognati gibi atipik yüz bulgular dikkati çekmiştir. Çekilen kranial MRI incelemesinde beyin sapı oluşumlarının ve frontal bölgede çok az beyin dokusunun mevcut olduğu, bunun dışında intrakranial kavitenin sıvı ile dolu olduğu görülerek hidransefali tanısı konmuştur. Aile izin vermediğinden otopsi yapılamamıştır. Hidrosefali, agria, retinal displazi, mikroftalmi ve ensefalosel gibi anomalilerle karakterli Walker-Warburg Sendromunda (WWS) bulgular geniş bir spektrum gösterebilmektedir. Olgumuzda hidransefali ve anoftalmi olması bu sendromun ağır bir formu gibi görülmekle birlikte mikrognati, basık burun kökü ve düz filtrum gibi atipik yüz bulgularının olması Walker-Warburg Sendromunu desteklememektedir.

Abstract

Hydrancephaly is a central nervous system malformation characterized by the almost completely absence of cerebral hemispheres. The second pregnancy of healthy unrelated parents (mother 20 years old) revealed semilobar holoprosencephaly on ultrasound examination at 38th week of gestation. Atypical facial appearance, with anophtalmi on the left and microphtalmi on the right eye with depressed nasal bridge, flattened philtrum and micrognatia were detected at birth. MRI revealed hydrancephaly with fluid-filled cavity and minimal brain tissue at frontal lobe and brain stem. Autopsy examination could not have been performed, because parents did not permit to be done. Walker-Warburg Syndrome (WWS) characterized by hydrocephalus, agyria, retinal dysplasia, microphthalmia and encephalocele, shows a wide spectrum of defects. Although our case can be suggested as a severe form of WWS with the presence of hydrancephaly and anophthalmia, the diagnosis is not supported because of the atypical facial appearance with micrognathia, depressed nasal bridge and flattened philtrum.
