Adenomyozis ve endometriozisde estrojen-progesteron reseptörü ( ER-PR ), Ki-67 antijen ve heat shock protein ( HSP ) ekspresyonları

Özet
Amaç: Bu çalışmanın amacı adenomyozis ve endometriosis ektopik endometriumu ile normal endometriumda ER ve PR, Ki-67 antijen ile HSP ekspresyonlarını saptamaktır. Yöntem: Arşivdeki dokulardan seçilen 40 adet adenomyozis ve endometrisis olguları ile adenomyozis olgularının endometriumları kontrol grubu olarak kullanıldı. İmmunhistokimyasal boyama HSP, Ki-67 (MIB1), ER ve PR monoklonal antikorlar kullanılarak yapıldı. (Kullanılan antikorlar ve dilüsyonlan sırasıyla : HSP: 1/250, Ki-67: 1/50, ER: 1/50, PR: 1/50) HSP için immünreaktivite; glandüler hücrelerin sitoplazmalarındaki boyanma yoğunluğuna göre, boyanma yok ise (-), zayıf boyanma (+) ve kuvvetli boyanma (++) olarak derecelendirildi. Proliferatif aktivite; 500-1000 hücre sayılarak boyanan hücrelerin boyanmayan hücrelere oranı hesaplandı. ER ve PR ; nükleer boyanmanın yoğunluğuna ve pozitif hücrelerin yüzde kombine edilerek üç gruba ayrıldı. İstatistiksel analiz SPSS (Statistical Package for Social Sciences) programı ile yapıldı. Sonuçlar: Ki-67 skoru, normal endometrium glandüler hücrelerinde endometriosis glandüler hücrelerine göre daha yüksektir (p=0.037). HSP, ER ve PR ekspresyonunda, normal endometrium ile adenomyosis ve endometriozis ektopik endometriumunda farklılık yoktur. Yorum: Bu sonuçlar, endometriotik dokuların büyük bir kısmının normal endometriumun siklik ER ekspresyonunu taklit etmesi sonucu olabilir ve endometriotik lezyonlarının, postmenopozal dönemdeki kadınlarda östrojen düzeyleri düşük olsa da hormonal olarak duyarlı kalıp, proliferatif aktivite gösterebilen lezyonlar olduğunun göstergesidir.
Abstract
Purpose; This study aimed to assess the expression of heat shock proteins, ER, PR, Ki- 67 antigen in eutopic and ectopic endometrium of endometriosis and adenomyozis. Methods: Forty cases of adenomyosis and endometriosis were selected from archival tissue and the endometrial tissue of the adenomyosis cases were used as the control group. Immunohistochemical staining was performed by HSP, Ki-67 (MIB1), ER and PR monoclonal antibodies (product and the dilutions: HSP: 1/250, Ki-67: 1/50, ER:1/50, PR: 1/50). immunoreactivity for HSP was observed in the cytoplasm of the glandular cells and the staining intensity was graded as (-) for no immunostaining, (+) for weak staining, and (++) for strong staining. Proliferative activity was quantified by counting 500-1000 cells and calculating the ratio of the stained to unstained cells. The ERand PR content was divided into three groups by combining the percentage of positive cells and the intensity of the nuclear stain. Statistical analysis was performed using the Statistical Package for Social Sciences. Results: Ki-67 score antigen was higher in the eutopic endometrial glandular component than in the glandular/epithelial component of the endometriosis (p=0.037). HSP, ER and PR expression was not different in eutopic and ectopic endometrium in both adenomyosis and endometriosis. Conclusions: These may be the result of the majority of the endometriotic tissues mimicked the cyclic ER expression of the eutopic endometrium. Endometriotic lesions appear to remain biologically active with proliferative activity as well as retained hormonal responsiveness, even in the low-estrogen milieu of the postmenopausal age.
