Primer epididimis leiomyosarkomu ( Olgu sunumu )

Özet

53 yaşındaki hasta sağ testisinde kitle yakınması ile Şanlıurfa Devlet Hastanesi Üroloji kliniğine başvurdu. Fizik muayenede sağ testiste üst pole lokalize sert, nodüler, ağrısız kitle saptandı. Hastaya sağ radikal orşiektomi ve yüksek kord ligasyonu yapıldı. Makroskopik olarak epididimin yerini alan, intakt testis dokusunu aşağıya doğru sıkıştıran sert, nodüler tümöral kitle görüldü. Tümörün histopatolojik incelenmesinde çok sayıda mitoz içeren sarkomatöz yapı izlendi. Immünohistokimyasal olarak aktin, desmin ve vimentin pozitif boyandı. Bu bulgular sonucunda tümöre primer epididimal leiomyosarkom tanısı konuldu. Son derece nadir görülen bir tümör olması nedeniyle olgu, literatür bilgileri eşliğinde klinikopatolojik ve immünohistokimyasal özellikleri ile tartışılarak sunuldu.

Abstract

A 53 year old male patient presented with a mass in his right testis to the urology clinic of Şanlıurfa Government Hospital. On physical examination, a nodular, firm mass located on the upper pole of testis was palpated. The patient was treated with a radical right orchiectomy and high cord ligation. On macroscopic examination, a firm, well circumscribed, white - gray nodular mass with a whorled cut surface was seen. The mass had pushed the intact testis tissue to the lower pole. Histopathologically, a sarcomatous tumor which consisted a lot of atypical mitotic figures was seen. The tumoral mass reacted positively with actin, desmin and vimentin. With these findings, a primary epididymal leiomyosarcoma was diagnosed. Since, these are rare neoplasms, we aimed to present and discuss this case with its clinicopathological and immunohistochemical aspects in the light of literature.
