Midede lenfoid follikül oluşumunun önemi, sıklığı, dağılımı, gastrit ve Helicobacter pylori ile ilişkisi

Özet
Lenfoid follikül oluşumu (LFO) île cinsiyet, peptik ülser, Helicobacter pylori (HP), gasrit ve lokalizasy onlar arasındaki ilişkiyi incelemek amacı île 133 semptomatik (55'i kadın ve 78'i erkek) olguya ait 267 mide endoskobik biyopsi örneği çalışmaya alındı. LFO izlenen olguların, %83'ünde HP pozitifliği saptandı, Bunların %52'sinde HP varlığı şiddetli derecede iken % 12'sinde hafif derecede bulundu. LFO'nün, olguların %36.5'inde korpusla ve %63.5'inde ise antrumda yerleştiği görüldü LFO sıklığı, gastrik ülser/erozyonlu olgularda %93, duodenal ülser/erozyonlu olgularda %52, aktif kronik gastritlilerde %36, kronik atrofik gastritlilerde %59 ve kronik gastritlilerde %44 olarak bulundu. HP kolonizasyon şekillerine göre LFO sıklığı mukus içinde serbest formunda %27, epitel yüzeyine adezyonda %25 ve interselüler kolonizasyonda %42 olarak saptandı. FO ile HP şiddeti, gastrit türleri, gastrik ülser/erozyon ve midedeki lokalizasyon arasında istatistiksel olarak oldukça anlamlı bir ilişki olduğu görüldü. Bu çalışma ile gastrik ülser/erozyonu olan ve/veya HP pozitifliği saptanan olgularda, LFO' nün özellikle aranması gerekliği ve bunun için doku örneğinin bitirilinceye kadar kesilmesi ve biyopsi örneklerinden en az birinin antrumdan alınmasının LFO' un saplanma oranım arttıracağı sonucuna varıldı.
Abstract
In these study, we examin2d 267 gastric endoscopic biopsy specimens from 133 symptomatic patients (55 females and 78 males) to elucidate relationship between formation ot lymphoid follicle (FLF) and sex, peptic ulcer, Helicobacler pylori (HP), gastritis and localizations. 83% of FLF cases were positive for HP. Among these, 52% of presence was wıth a high degree whereas 12% of them were with a low degree, FLF was observed to be localized İn corpus (36.5%) and antrum (63.5%). PLF incidence in patients, were as follow: gastric ulcus/erosion (93%), duodenal ulcus/erosion (52%), active chronic gastritis (36%), chronic gastritis (44%), chronic airophic gastritis (59%) and HP colonization modes (free in mucus: 27%, surface-adhesion: 25% and intercellular colonization: 42%). We found atatislicaly significant differences between FLF and gastric ulcus/erosion, localization. degree of HP and forms ol gastritis. These results indicate that FLF must be searched in patients with gastric ulcus/erosion and/or HP positive. At least one of gastric biopsy specimens must be taken in antrum and cut until it is finished
