Özet

Kısa barsak sendromunda barsak emilim yüzeyini arttır¬mak amacıyla intestinal neomukoza oluşturulması seçene¬ği üzerinde değişik yöntemler geliştirilerek araştırmalar sürdürülmektedir. Intestinal mukozanın yamanan yüzey¬lerde büyüdüğünün gösterilmesine karşın, bu yüzeylerin kontraksiyonu halen büyük bir sorundur. Bu araştırmanın amacı, alternatif model oluşturabilmek için, mideden ha¬zırlanan pediküllü seromüsküler yama üzerinde intestinal neomukoza oluşumunu göstermek ve bu yöntemi kısa bar¬sak sendromunda kullanılan benzer yöntemler ile karşılaş¬tırmaktır. Bu amaçla otuz Wistar Albino cinsi sıçan iki gruba ayrıldı; birinci grupta kullanılan yama ileuma, ikin¬ci grupta ise jejunuma anastomoze edildi. Histopatolojik incelemeler sonucunda yama yüzeyinin ileum grubunda % 542'sinin, jejunum grubunda ise % 50.6'sının neomukoza ile kaplandığı saptandı. Her iki grupta da fibroblastik ak-tivite artışı, kollajen artışı ve enflamasyona bağlı olarak, ileum grubunda yama yüzey alanının % 64.5'ine, jejunum grubunda ise yama yüzey alanının % 58.6'sına inerek kontraksiyona uğradıkları saptandı. Ancak ileum ve jeju¬num grupları arasında kontraksiyon açısından saptanan bu fark istatistiksel olarak anlamlı bulunmadı (p>0.05). Bu çalışmada saptanan kontraksiyon oranlaranın alterna¬tif yöntemlere oranla daha az olması ve kullanılan yüzey üzerinde de intestinal neomukozanın geliştiğinin gösteril¬mesi, mideden hazırlanan pediküllü seromüsküler yama¬nın kısa barsak sendromunun cerrahi tedavisinde neomu¬koza oluşturan yöntemlere bir alternatif olabileceğini ka¬nıtlamaktadır.

Abstract

Gastric seromuscular patch for intestinal neomucosal growth Growth of neomucosa has been investigated in regard to increase the intestinal surface area in short-bowel syndro¬me. Functional neomucosa grows over patched intestinal defects, but the contraction of the grafts remains still as a problem. The aim of this study was to evaluate the neomu¬cosal growth on the pedicled gastric patch over the small intestine and to compare the apliciability of this technique in the treatment of short bowel syndrome in regard to in¬testinal neomucosal growth. 30 Wistar Albino rats were divided into two groups; Group I: jejunal group (n=15), group II: Heal group (n=15). In group I a jejunal segment and in group II an Heal segment were patched with pe-dicled gastric seromuscular patch. These patched intesti¬nes were investigated after 6 weeks histopathologically. The defects were covered with neomucosa in both patched groups (in Heal group: 54.2 %, in jejunal group: 50.6 %). However in both groups fibroblastic activity, increase in collagen production and inflamation caused a contraction of the patched swface area to a certain degree (64.5 % in Heal group and 58.6 % in jejunal group). The difference between these two groups was not statistically significant. In this study intestinal neomuscosal growth was demonst¬rated over the gastric patch and the rate of contraction in patched flaps was lower than the alternative methods. We conclude that this technique could be useful in short bowel syndrome in regard to intestinal neomucosal growth.
