Apandisit düşünülen olgularda laboratuvar testlerinin tanısal değeri: Prospektif bir klinik çalışma

Özet

Akut apandisit düşünülen olgularda negatif laparotomi oranını azaltabilmek amacıyla C-Reaktif Protein (CRP), lökosit sayısı ve nötrofil yüzdesinin tanısal değerlerinin araştırılması amaçlanmıştır. Kasım 1997-Ocak 1999 arasında kliniğimizde akut apandisit ön tanısıyla ameliyat edilen 105 hastanın başvuru anında kan örnekleri alınarak CRP, lökosit sayısı ve nötrofil yüzdeleri çalışıldı. Olguların yaşı, cinsiyeti, semptomlarının başlangıç süreleri (12 saatten önce veya sonra) ile makroskopik (normal apandiks, akut apandisit) ve histopatolojik bulgular kaydedildi. Veriler; testin tahmin değeri, duyarlılık ve seçicilik açısından hem tek başına hem de kombine edilerek analiz edildi. Standart laboratuvar referanslar ile, en yüksek duyarlılık (% 84) nötrofil yüzdesinde, en yüksek pozitif tahmin değeri (% 94) ise lökosit sayısında elde edildi. Her üç test 'veya' kuralı ile kombine edildiğinde negatif laparotomiler için duyarlılık % 100'e ulaştı. CRP' nin sentezi için belirli süre geçmesi gerektiği düşünülerek hesaplama 12 saatten önce ve sonra başvuranlar şeklinde bölündüğünde duyarlılığın % 43'ten % 83'e, pozitif tahmin değerinin % 87'den % 93'e yükseldiği bulundu. Tüm testlerden elde edilen değerlerin ortalamaları kendi içlerinde, apandisit olup olmamasına göre karşılaştırıldığında, sadece lökosit sayısı anlamlı bulundu (p<0.05). Testlerin tek başlarına veya kombine kullanımının apandisit tanısı konulmasında sınırlı değeri vardır. Buna karşın üçlü test apandisit tanısını dışlamada % 100 seçiciliğe sahiptir. Bu nedenle, apandisit düşünülen olgularda üç testin birlikte kullanımının negatif laparotomi oranını düşürebileceği kanısındayız.

Abstract

Aim of this study is to determine the diagnostic value of C-Reactive Protein (CRP), white blood-cell count (WBC) and neutrophil percentage (NP) in cases of suspected appendicitis to lower the rate of negative laparotomies in children. From November 1997 to January 1999, CRP, WBC and NP were determined in blood samples obtained at admission from 105 patients operated with a presumptive diagnosis of acute appendicitis. Age, gender, duration of symptoms (within 12 hours or longer), macroscopical and histopathological (inflamed or uninflamed appendix) data of patients were recorded. Data were analyzed to determine the predictive values, sensitivity and specificity of single and combined tests. Using standard reference ranges, the best sensitivity was observed in NP (84 %), whereas highest positive predictive value was found in WBC (94 %). When the three tests were combined by an "or" rule, the sensitivity for negative laparotomies was enhanced to 100 %. When CRP values were analyzed according to the onset of symptoms as within 12 hours or more (due to its time consuming synthesis), sensitivity increased from 43 % to 83 %, and positive predictive value from 87 % to 93 %. When the data from the tests were compared within themselves depending on whether appendicitis was present or not, only WBC was significant (p<0.05). The tests have limited value in the diagnosis of acute appendicitis, used either alone or in combination. However, when used in combination, these three tests are valid in excluding the diagnosis of acute appendicitis, with 100 % specificity. Thus, the application of the triple test could be of help in reducing the incidence of negative laparotomies in cases of suspected appendicitis.
