Çocuklarda ambigius genitalyanın cerrahi tedavisi

Özet
Amaç: Ambigius genitalylı çocuklarda cinsiyetin belirlenmesi, doktor ve aileler için halen önemli bir sorundur. Bu çalışmada kliniğimizde uygulama bulan tedavi yaklaşımının değerlendirilmesi amaçlanmıştır. Yöntem: 1990-1996 yıllan arasında ambigius genitalya tanısı ile ameliyat edilen 27 hastaya ait veriler geriye dönük olarak nicelendi. Bulgular: Olgulardan 11'i dişi psödohermafrodit, 11I'i erkek psödohermafrodit, 4'ii gerçek hermafrodit ve 1'i mikst gonadal disgenezi tanısı aldı. Tanı yaşlan 8 gün ile 16 yıl (ort. 3.7 yıl) arasında değişmekteydi. Dişi psödohermafrodit olgulardan, 8'ine sadece kliteroplasti, 2'sine kliteroplasti+periııeal vajinoplasti ve birine de klitero-plasti+pull-throııgh vajinoplasti uygulandı. Onbir erkek psödohermafrodit olgunun 6'sına bilateral orşiektomi yapılarak kız cinsiyeti yönünde ameliyat edildi. Bu 6 olgunun 2'sine vajinoplasti yapıldı. Bu gruptaki 5 olgu ise orşidopeksi ve hipospadias onarımı ile erkek cinsiyeti yönünde ameliyat edildi. Gerçek hermafrodit 4 olgudan 3'üne erkek, birine ise kız cinsiyeti yönünde girişim uygulandı. Mikst gonadal disgenezi tanısı alan 1 olguya bilateral gonadektomi ve vajinoplasti yapıldı. Sonuç: Ambigius genitalyalı olgularda cinsiyet değişikliği veya belirlenmesindeki gecikme, ciddi cinsel kimlik bozukluklarına neden olabilmektedir. Bu yüzden ambigius genitalya, erken tanı ve tedavi gerektiren acil bir problem olarak kabul edilmeli ve tedavi buna uygun planlanmalıdır.
Abstract
Background/Aim: Gender assignment in children with ambiguous genitalia is still a problem for the parents and physician. in this study we aimed to evaluate the current management strategy in our department. Methods: We reviewed the outcome of 27 children with ambiguous genitalia which had been managed surgically in our clinics between 1990 and 1996, retrospectively. Results: This series consists of 11 female pseudohermaphrodites with congenital adrenal hyperplasia", 11 female pseudohermaphrodites,4 true hermaphrodites and I child with mixed gonadal dysgenesis. The age at diagnosis ranged from 8 days to 16 years (mean 3.7 years). Eight patients with female pseudohermaphroditism were man-aged with clitoplasty alone, two with cliteroplasty and perineal vaginoplasty, 1 with cliteroplasty and pull-through vaginoplasty. Six of 11 cases of male pseudohermaphrodites were given a female sex assignment; and underwent bilateral orchidectomy. Two of these 6 patients required vaginoplasty. Five children were raised as males and managed with orchidopexy and hypospadias repair. There were 4 true hermaphrodites of whom 3 were assigned male gender roles, one was assigned female gender role. One case of mixed gonadal dysgenesis was given female sex assignment and managed with bilateral gonadectomy and vaginoplasty. Conclusion: The management of the child with ambiguous genitalia continues to be one of the more challenging issues. Serious problems of gender identity result when sex assignment is changed or delayed. Sexual ambiguity, therefore, should be considered an urgent social problem that demands early evaluation and surgical treatment.
