Deneysel üreter obstrüksiyonunun açılması sonrası böbrekte gelişen iskemi-reperfüzyon hasarının incelenmesi

Özet
Önbilgi!Amaç: Akut üreter obstrüksiyonu sonrası meyda-na gelen renal hasardan, obstrüksiyon dönemi kadar, obstrüksiyonun açılmasını izleyen reperfüzyon döneminin de sorumlu olabileceği düşünülmektedir. Çalışmamızda sıçanlarda 24 saatlik tek taraflı üreter obstrüksiyonu oluşturarak, iskemi-reperfüzyon hasarının belirlenmesi amaçlanmıştır. Yöntem: İskemi repeıfüzyon hasarının tespiti için lipid peroksidasyonu ürünleri olan TBARS ve okside olmamış protein sülfidril gruplarını gösteren okside olmamış proeinlerin sulfidril gruplarının (RSH) doku düzeyleri belirlenmiş ve ilgili renal vende BUN ve kreatinin düzeylerine bakarak böbrek işlevindeki etkiler araştırılmıştır. Oluşması muhtemel hasar da aUopurinol ve trasylol ile önlen-meye çalışılmıştır. Bulgular: Çalışmada grupların doku TBARS düzey ortalamaları arasında bir fark bulunamadı (p>0.05), allopurinol ve trasylol ile tedavi edilen gruplarda ise RSH düzeyleri ortalamaları anlamlı olarak yüksek bulundu (p<0.005). Böbrek işlevlerine yönelik değerlendirdiğimiz BUN ve kreatinin gibi parametrelerde gruplar arasında anlamlı bir fark bulunmadı. Sonuç: Bu bulgular bize üreter obstrüksiyonu sonrası serbestleştirme döneminde belirgin iskemi-reperfüzyon hasarının olmadığını göstermektedir.
Abstract
BackgrounlAim: For renal injury occurring during acute ureter obstruction it is thought that, the reperfusion period following the release of the obstruction is as responsible as the obstruction period. in this study a 24 hour uni-lateral acute ureter obstruction was performed in rats and the ischemi-reperfusion injury was investigated. Methods: We investigated the ischemia-reperfusion injury by evaluating the tissue levels of TBARS, a lipid peroxidation product and RSH, a nonoxidated protein sulfhidryl group. Blood samples were taken from the ipsilateral renal vein and serum BUN and creatinine levels were measured to evaluate renal functions. The expected ischemia-reperfusion injury was attempted to be prevented with aUopurinol and trasylol. Results: in this study no significant difference was identi-fied among the average of tissue TBARS levels of the gnntps (p>0.05). in the low dose aUopurinol and trasylol treatment groups, the average of the RSH levels were found to be significantly higher (p<0.005). Conclusion: We can conclude that, after the release of acute ureter obstruction, the ischemia-reperfusion injury is not significant.
