Özet

Amaç: Kliniğimizde invajinasyon tanısı ile tedavi edilen hastaları başvuru semptomları, invajinasyon sebepleri, tanı-tedavi yöntemleri ve komplikasyon açısından değerlendirmek. Yöntem: Ocak 1991-Ocak 2000 tarihleri arasında invajinasyon nedeniyle tedavi edilen hastaların dosyaları, geriye dönük olarak yaş, semptomatoloji, tanısal yöntem, tedavi yöntemi ve ameliyat bulguları açısından incelendi. Bulgular: Toplam 53 hastanın (1 ay-14 yaş, ort. 1.5 yaş) % 66'sı (35/53) 2 yaşın altındaydı ve bunların % 40'ı invajinasyon tanısı konulduğunda ÜSYE tedavisi görmekteydi. En sık rastlanılan semptom kolik tarzı karın ağrısı idi. 2 yaş altındakilerin hepsinde rektal kanama vardı. Hastaların % 26'sı 4 yaşın üzerindeydi ve bunların % 71.4'ünde invajinasyona yol açan bir leading point bulundu. Toplam 13 olguda leading point saptandı. Toplam 43 hastaya ultrasonografi yapıldı ve hepsinde de invajine barsak segmenti görüldü. Baryumlu redaksiyon % 74 (39) hastada uygulandı. Redaksiyonların % 59'u (23 hasta) başarılıydı, bu hastaların da % 70'i 1 yaşın altındaydı. Redaksiyon sonrası 4 hastada (% 10) reinvajinasyon, 1 hastada (% 2.5) perforasyon gelişti. Baryum redaksiyonunun başarısız olması nedeniyle opere edilen 16 hastanın 4'üne rezeksiyon-anastomoz (% 25), 12'sine manuel redüksiyon (% 75) yapıldı. Manuel redaksiyon yapılan hastaların 2'sinde (% 5) postoperatif reinvajinasyon görüldü. 14 invajinasyon hastasına sadece USG ile tanı konularak ameliyata alındı. 2 HSP'lı hastanın birine rezeksiyon anastomoz yapılırken, diğerinin invaginasyonunun spontan olarak düzeldiği bulundu. Üç hastada postoperatif invajinasyon nedeniyle manuel redaksiyon yapıldı. Dokuz hasta ise peritonit bulguları nedeniyle operasyona alınarak rezeksiyon anastomoz yapıldı. Mortalite görülmedi. Sonuç: 2 yaş altındaki olgularda, hidrostatik redaksiyon denenmesi büyük oranda başarılı sonuç verebilir. Daha büyük yaşlarda, leading point varlığının yüksek oranı gözönünde bulundurularak, hidrostatik redaksiyonda çok ısrarlı davranmamak ve tedavi planını bu yüksek orana göre oluşturmak yerinde bir davranış olacaktır.

Abstract

Aim: To evaluate children treated for invagination in our clinic with regard to symptoms at admission, whether the invagination was idiopathic or secondary to a lead point, methods of diagnosis-treatment and complications. Method: Records of children with invagination between years 1991-2000 were evaluated with regard to age, sex, invagination type, symptoms, signs, mode of diagnosis, treatment and complications. Results: Among 53 children (age: 1 month-14 years, average age: 1.5 years) 66 % of cases were under 2 years of age, 40 % of which were receiving therapy for upper respiratory tract infection. The most common symptom was colicy abdominal pain. All of the children under 2 years of age had rectal bleeding. 26 % of the patients were older than 4 years, 71.4 % of which had leading points. A total of 13 cases had leading points. USG demonstrated the invaginated segment in all of the 43 patients it. Hydrostatic reduction was sattempted in 74 % (39) of patients and was successful in 59 % (23) of which 70 % was under 1 year of age. Following hydrostatic reduction, 4 (10 %) reinvagination and 1 (2.5 %) perforation was seen. Of 16 cases with unsuccessful hydrostatic reduction attempt, in 4 cases intestinal resectionprimary anastomosis (25 %), and in 12 (75 %) manual reduction were done. In 2 (5 %) of the manual reduction patients, reinvagination was seen. 14 patients were diagnosed with USG only and operated. Of these, while 1 of 2 HSP patients required an intestinal resection and anastomosis, the other was found to have his invaginated segment reduced spontaneously at operation. Three of these 14 patients had postoperative invagination reducted manually. Nine of the patients had signs of peritonitis at admission and were treated by resectionprimary anastomosis. No mortality was encountered in this series. Conclusion: In cases of invagination under 2 years of age, hydrostatic reduction should prove to be succesful. In older children, considering, the high incidence of leading points, one should not insist on hydrostatic reduction but should rather design a treatment plan accordingly.
