Fetal akciğer matürasyonunun değerlendirilmesinde lamellar cisim sayımının, L/S ve sürfaktan/albumin oranı ile karşılaştırılması

Özet
Amaç: Çalışmamızda hızlı sonuç verebilen ve kolay yapılan bir akciğer matürasyon testi olan amnion sıvısında lameller cisim sayımının (LCS) güvenirlik, avantaj ve dezavantajlarını ortaya koymaya çalıştık. Çalışma Şekli: 1.1.1995-1.6.1995 tarihleri arasında SSK Göztepe Eğitim Hastanesi, 4. Kadın Hastalıkları ve Doğum Kliniğinde gebelik komplikasyonları sebebiyle hospitalize edilmiş fetal matürite tayini gereken 31 gebeye amniosentez uygulanmıştır. Alman örneklerde LCS, L/S oranı ve floresan polarizasyon metodu ile sürfaktan / albümin oranı (TDxFLM) saptanmıştır. Ayrıca tüm gebelikler olası postpartum respiratuar distres sendromu (RDS) açısından izlenmiştir. Bulgular : LCS ile TDxFLM testi arasında ileri derecede anlamlılık (p<0.001), iyi derecede korelasyon (r=0.67) saptandı. LCS ile L/S değerleri arasındaki korelasyon zayıf olarak bulundu. TDxFLM ve L/S oranları arasında anlamlılık p=0.035, korelasyon (r-0.49) orta derecede bulundu. Amniosentez sonrası 72 saat içinde gerçekleşen doğumlar dikkate alınırsa 27-39. haftalarda 1100-3300 gram fetal ağırlığa sahip 16 canlı bebek doğurtulmuş olup, iki yeni doğanda RDS saptandı. Bu bebeklerde LCS değerleri 5.000/cc ve 12.000/cc, TDxFLM 13.37 mg/gr ve 11.4 mg/gr idi. Diğeryeni doğanlarda LCS değerleri 31.000/cc ile 217.000/cc arasında idi. LCS değerleri yüksek olan bu olgularda L/S oranları 2 'nin üstünde, TDxFLM değerleri 50mg/gramın üstünde idi. Sonuç: Çalışma sonunda LCS'nin fetal pulmoner matürite tayininde diagnostik gücünün yüksek ve bulunan değerler ile referans olarak kullandığımız TDX-FLM (floresans polarizasyon metodu) değerleri arasında anlamlı bir korelasyon olduğu (r=0.67, p=0.001) tespit edildi.
Abstract
Objective: We tried to find out the reliability, advantages and disadvantages of a rapid and simple lung maturity test called lamellar body count ( LBC). Study Design : From 1.1.1995 to 1.6.1995, 31 amniocentesis were performed on pregnant patients requiring fetal lung maturity determination, and hospitalised for complication of pregnancy at the SSK Educational Hospital , Department of Ob/ Gyn. Withdrawn samples were sent to laboratory for determination of LBC, L/S ratio and surfactant/ albumin ratio detection by fluorescent polarisation method (TDxFLM). All pregnancies were followed up for possible postpartum development of respiratory distress syndrome (RDS). Findings : There was a good correlation (r= 0.67) between the results of LBC and TDxFLM tests and it was found to be statistically important ( p< 0.001 ). There found to be a weak correlation between LBC and L/S ratios. ' TDxFLM and L/S ratios were found to be moderately correlated ( r = 0.49, p = 0.035 ). When we consider the deliveries occurring within 72 hours after the amniocentesis, there were 2 RDS among 16 alive new-boms at the 27 toi 39 weeks of gestation and weighing between 1100 and 3300 grams. These two new-boms' LBC values were 5.000/cc and 12.000/cc, TDxFLM values were 13.37 mg/gr and 11.4 mg/gr, respectively. The rest of the babies had , LBC values between 31.000/cc and 217.000/cc, and all of them had L/S ratios > 2 and TDxFLM values> 50 mg/g. Conclusions LLBC was discovered to be a good diagnostic tool, and it has a strong correlation with gold standard test named TDxFLM (r = 0.67, p< 0.001).
