Adneksial kitlelerin ayırıcı tanısında Doppler sonografinin yeri CA-125 ve ultrasonografi ile karşılaştırılması

Özet
Cerrahi operasyon öncesi, toplam 53 adnekstyel kitle ayırıcı tanı yönünden Dopler sonografi, Serum Ca-125 ve transvaginal ultraso¬nografi ile değeriendUHdtPerüeral rezistansı değerlendirmek için rezistans indeks (Rl) cut-ojf değerinin 0.41 alındığı araştırmada in-tratümöral kan akımı malign kulelerin tamamında (%100) saptanırken, benign kitlelerin 9650'sinde saptandı. Serum Ca-125 cutoff değeri ise 35 U mi alarak alındı. Ri, Ca-125 ve TV ultrasonografinin sensitivtte, spesifite, negatif ve pozitif tahmini değerleri hesap¬landı. Sonuç olarak adnekstyel kiuelerin ayına tanısında, Doppler sonograflnin, TV ultrasonografi ile aynı senşittviteye fakat USG ve Ca-125'den daha fazla spesifiteye sahip elmasına rağmen her üç parametrenin birlikte kullanılmasıyla daha iyi sonuçlar elde edileceği görüşündeyiz.
Abstract
Prior to surgery, to evaluate 53 adnexlal masses, Color Doppler Sonography, Serum Ca-125 and gray-scale ultrasonography were used. Resistans index having the cutoff point of 0.41 was used to determine the peripheral resistance of intratumoral vessels. The blood flow was delected in 100% of malignant tumors and 50% of benign masses. The cutoff value for Ca-125 was 35 U/mL Sen¬sitivity, specificity, positive and negative predictive values were compared in terms of Rt, Serum Ca-125, gray-scale sonography sco¬ring system. Our conclusion is that, to evaluate adnexlal masses. Color Doppler Sonography has the same sensitivity with gray-scale USG, but has more spectuictty than Serum Ca-125 and Ultrasonography. As a result the combination of each parameters will be more effec¬tive method.
