Özet

Kliniğimizde Ocak 1992-Ekim 1996 tarihleri arasında 33 hastaya 36 kapak rereplasman ameliyatı uygulanmıştır. 30 hastaya 2. kez, 3 hastamız ise 3. kez kapak rereplasmam için ameliyat edilmiştir. 36 rereplasman ameliyatının 20'si mitral, 16's aortik pozisyonda uygulanmıştır. 5 hastamıza Triküspit De Vega Anuloplasti ek cerrahi girişim olarak ilave edilmiştir. Ameliyat endikasyonlan içinde en sık primer kapak disfonksiyonu yer almaktadır. Postoperatif erken dönemde 4 hastamız exitus olmuş, 3 hastamız da kanama nedeniyle reoperasyona alınmıştır. Geç dönem takiplerimizde 2 hastamız exitus olmuş, 2 hasta primer kapak disfonksiyonu ve 1 hastada bakteriyel endokardit nedeniyle 3. kez kapak rereplasman ameliyatı uygulanmıştır. Postoperatif 6. ayda yapılan değerlendirme sonucunda hastaların preoperatif değerlerine göre fonksiyonel kapasitelerinde belirgin düzelme olduğu görülmüştür. Redo ameliyatlarda cerrahi tecrübenin de artmasıyla postoperatif mortalite ve morbiditenin minimale ineceği inancındayız.

Abstract

33 patients underwent 36 valvular rereplacement operations between June 1992 and October 1996 in our clinic. 30 patients were operated for the second and 3 patients were operated for the third time for valvular rereplacement. 20 of the 36 rereplacement operation were performed in mitral , 16 were performed in aortic position. 5 of our patients were accompanied with the De Vega annuloplasty of the tricuspid valve as the additional surgical intervention. Primary valvular dysfunction is the commonest indication for the operation. In the early postoperative period, 4 patients died and 3 patients underwent reoperation for bleeding. In our late period follow-up, 2 patients died, 2 patients underwent valvular rereplacement operation for primary valvular dysfunction and 1 patient for the infective endocarditis for the third time. The results of the postoperative svcth month reveal, the functional capacities of the patients improved to preoperative values. We believe that postoperative mortality and morbidity in the re-do operations will be minimal by increasing surgical experience.
