Özet

AMAÇ: Alt ekstremite bypass greftlerinde iki boyutlu time-of-flight (2D TOF) manyetik rezonans anjiografi (MRA) 'nin tanısal değerinin belirlenmesi amaçlanmıştır. GEREÇ VE YÖNTEM: Bölümümüze alt ekstremite bypass greft oklüzyonu düşünülerek gönderilen 5 olguya dijital subtraksiyon anjiografi (DSA) ve MIH tetkikleri yapıldı. MRA incelemesi 1.5-T Gyroscan MR üniti (Philips Medical Systems) ile 2D TOF (TR/TE 23/5.4 ms, FA 20°-60°) tekniği ile abdominal aorta distalinden trifurkasyona kadar üç basamak halinde 4 mm'lik 100'er kesit alınarak yapıldı. Abdomen ve uylukla vücut koili, diz bölgesinde diz koili kullanıldı. Beş olgudaki total 24 seğmeni (distal abdominal aorta, ana ve eksternal iliak arterler, ana ve yüzeyel femoral arterler, popliteal arter, infrapopliteal arterler -bir segment; tibialis anterior-) değerlendirildi. Sonuçlar normal, stenoz ve oklüzyon olarak sınıflandırıldı. BULGULAR: Her iki teknikle 15 segment normal, bir segment hafif stenoz ve 8 segment tam oklüzyon olarak değerlendirildi. MRA'nın sensitivitesi ve spesifitesi % 100 olarak bulundu. SONUÇ: Alt ekstremite bypass greftlerinde noninvaziv bir inceleme metodu olarak MRA. yüksek sensitivile ve spesifite ile kullanılabilir.

Abstract

PURPOSE: To determine the diagnostic efficacy of two dimensional time-of-flight (2D TOF) magnetic resonance angiography (MRA) in lower extremity bypass grafts. MATERIALS AND METHODS: Both DSA and MRA were performed in 5 patients, who were transferred lo Radiology Department with the suspicion of lower extremity bypass graft occlusion. MRA examination was done with 1.5 -T Gyroscan MR unit (Philips Medical Systems) by using 2D TOF (TR/TE: 23/5.4 ms, FA 20°-60°) technique starting from distal aorta to the trifurcation in three steps, 100 slices were taken, each slice 4 mm. Body coil for abdomen and thigh, knee coil for the knee were used. A total of 24 segments (distal part of abdominal aorta, common iliac, external iliac, common femoral, superficial femoral popliteal and infrapoplileal arteries one segment; anterior tibial-) in five patients were evaluated with both methods. The results were categorized as normal, stenosis and occlusion. RESULTS: Fifteen segments were normal, slight stenosis was detected in one and complete occlusion in 8 in both techniques. The sensitivity and specificity of MRA were 100 %. CONCLUSION: MRA, being a non-invasive method, can be used in lower extremity bypass grafts with very high sensitivity and specificity rates.
