Akut immun trombositopenik purpura: Yüksek doz intravenöz metilprednizolon tedavisi ile farklı dozlarda uygulanan intravenöz immünglobulin tedavilerinin karşılaştırılması

Özet
AMAÇ: Bu çalışma Akut İdiopatik Trombositopenik Purpura (İTP) tedavisinde farklı dozlardaki intravenöz immünglobulin (IVIG) tedavisi ile yüksek doz intravenöz metilprednizolon (YDMP) tedavi protokollerinin değerlendirilmesi amacıyla planlandı. YÖNTEM: Klinik ve laboratuvar bulguları ile akut İTP tanısı konulan 34 olgu rastgele iki gruba ayrıldı. Grup 1'de 12 olguya YDMP uygulandı (30 mg/kg/gün, 3 gün süre ile, ardından 20 mg/kg/gün 4 gün süre ile). Grup 2'deki 22 olgunun hepsine başlangıçta düşük doz IVIG (250mg/kg/gün, 2 gün süre ile) uygulandı. Yeterli cevap alınamayan 19 olguda bir üst doz olan, orta doz IVIG (400mg/kg/gün, 2 gün süre ile) tedavisine geçildi. Bu doz ile de cevap alınamayan 8 olguya ise yüksek doz IVIG (1gr/kg/gün, 2 gün süre ile) uygulandı. BULGULAR: YDMP tedavi protokolü tedavinin etkinliği bakımından İVİG tedavi protokolüne göre daha başarılı bulundu (p 0,05). En iyi sonuç orta doz IVIG tedavi protokolü ile elde edildi. YDMP ve İVİG tedavi protokollerinde tedavinin kesilmesini gerektirebilecek önemli bir yan etki saptanmadı. SONUÇ: Çalışmamızın sonuçları akut İTP tedavisinde intravenöz YDMP 'un IVIG 'e göre daha etkili olduğunu telkin etmektedir.
Abstract
AIM. This study was designed to evaluate the treatment protocols of high dose intravenous metilprednisolone (HDMP) and different doses of intravenous İmmünglobulin (IVIG) in the treatment of acute immun trombocytopenic purpura (ITP). METHOD: Thirty-four cases who were diagnosed as acute İTP by laboratory and clinical findings were randomly divided into two groups. HDMP was administrated in 12 cases in group 1 (30 mg/kg/day, for three days followed by 20 mg/kg/day, for four days). Low dose IVIG (250 mg/kg/day, for two days) was administrated in all 22 cases in the second group. A moderate dose of IVIG (400 mg/kg/day, for two days) was administrated in 19 cases with inadequate response. A high dose of IVIG (1gr/kg/day, for two days) was administrated to 8 cases out of 19 with inadequate response. RESULTS: It has been determined that HDMP treatment protocol was more successful that IVIG with regard to the effectiveness of the treatment (p <0,05). In the evaluation of the success of higher doses of IVIG, it has been determined that transition from low dose to moderate dose was successful (p<0,05), but the transition from moderate dose to high dose was not successful (p>0,05). The best response was obtained with moderate dose IVIG treatment protocol. No significant adverse effect that would require stopping the treatment was observed in HDMP and IVIG treatment protocols. CONCLUSION: the results of this study suggest that HDMP treatment protocol is more effective than IVIG protocol in the treatment of ITP.
