Özet

AMAÇ: Akut böbrek yetmezliği (ABY) sendromu farklı etyolojik sebeplerle ortaya çıkan, her yaşta görülebilen ve prognozu kötü olan bir tablodur. Çalışmamızda ABY prognozunun yaş ile ilişkisini ve bu ilişkiye etkili faktörleri ortaya koymayı amaçladık. YÖNTEM: Kliniğimizde takip edilen 102 ABY tanısı almış hasta retrospektif incelendi. Hastalar yaşlarına göre dört gruba ayrılarak mortalite, etyoloji, komplikasyon, oligürik seyir ve diyaliz ihtiyacı karşılaştırıldı. Ayrıca ölen ve sağ kalanların; yaşları, yaş-serum total protein (TP) ve yaş-günlük kan üre azotu (BUN) artış hızı korelasyonu incelendi. BULGULAR: Ortalama yaş; ölenlerde 48.26±l6yıl (22-77), sağ kalanlarda 38.51±17.8 yıl (14-85) idi (p<0.05) 40 yaş üzerindeki Grup 3 ve 4'deki mortalite oranları, 25 yaş ve altındaki Grup 1 'e göre fazla idi (p<0.05). Onuncu gün serum total protein (TP); ölenlerde 5.32±0.87 g/dl, sağ kalanlarda 6.45±0.86 g/dl idi (p<0.0001). Yaş ile TP arasında negatif korelasyon vardı (p<0.05, r2=0.36). Gruplar arasında etyolojik farklar vardı. Sepsis komplikasyonu yaşla birlikte artmaktaydı. SONUÇ: Yaşlı hastalarda prognozun kötüleştiğini saptadık. Mortalitede etyoloji, komplikasyonlar ve takip sırasında plazma TP düzeyinin düşmesi etkili faktörlerdi. Diyaliz ihtiyacı tüm hastalarda kötü prognoz işaretiydi.

Abstract

AIM: Acute renal failure (ARF) is one of the bad prognostic diseases, which can be seen in every age groups with different etiological reasons. In our study, we planned to explore the effect of age on the prognosis of ARF and the factors influencing this relationship. METHOD: 102 patients diagnosed as ARF in our clinic were evaluated retrospectively. Patients were separated into four groups according to their ages and compared with each other according to their mortality, etiology, complications, oliguric course and need of dialysis. In addition age of the survived and death patients, serum total protein levels, rate of increase in daily blood urea nitrogen (BUN) levels were investigated. RESULTS: Average age for the patients who died was 48.26±16 (22-77) years, it was 38.51±17.8 (14-85) years for -the survived ones (p<0.05). Serum total protein (TP) level was 5.32±0.87g/dl for the death patients and it was 6.45±0.86g/dl for the survived patients (p<0.0001). There was negative correlation between age and TP levels (p<0.05, r2 = 0.36). There was etiological differences between the groups. Sepsis rate were observed to be increased with the increasing age. CONCLUSION: Prognosis were seemed to be worse with advanced age. Etiology, complications and low levels of TP during follow up were the important factors. Need of dialysis was the sign of bad prognosis for all of the patients.
