Özet
AMAÇ: Bu çalışmada hiperbilirubinemi-nedeni ile kan değişimi yapılan çocukların uzun dönem takipleri ile hiperbilinıbineminin ve kan değişiminin risklerinin değerlendirmesi amaçlandı. YÖNTEM: İndirekt hiperbilirubinemi tanışı ile kan değişimi uygulanan 49 vaka çalışmaya alındı. Olguların fizik ve nörolojik muayeneleri yapıldı. Hemogrâm, karaciğer fonksiyon testleri, hepatit B ve C'ye ait belirleyicileri araştırıldı. BULGULAR: Hastaların ortalama takip süresi 3,5 ± 1,7. yıldı. En sık karşılaşılan kan değişimi nedeni ABO uyuşmazlığı (% 51), takiben Rh uyuşmazlığı (% 10,2) idi. Beş olguda (%10,2) nörolojik sorun saptandı. % 14,3 oranında demir eksikliği anemisi saptandı. Büyüme parametreleri kontrol grubundan farklı değildi. Tüm vakalarda anti-HCV ve HBs-Ag negatif bulundu. SONUÇ: Uzun dönem takipler ile indirekt hiperbilinıbineminin ve kan değişimine ait komplikasyonların etkilerinin tesbiti mümkündür. Donör kanlarında yapılan etkin taramalar post-transfiizyon hepatit gelişme riskini azaltacaktır.
Abstract
AIM: In this study long-term follow-up of babies who were applied exchange transfusion and evaluation of risks of hyperbilirubinemia and exchange transfusion were aimed. METHODS: Forty-nine babies who applied exchange transfusion due to indrect hyperbilirubinemia were recruited, for the study. Physical and neurologic examination of patients were done. Blood counts, liver function tests, markers for hepatitis B and C of patients were investigated. RESULTS: Mean follow-up time of pattern was 3.5±1.7 year. The most encountered cause of exchange transfusion was ABO blood goup incompatibility (%51), second was Rh incompatibility (%10.2). Five patients have neurologic problems. %14.3 of them have iron deficiency anemia. Growth parameters of patients were not different from control group. Anti-HCV andHbsAg were found negative for all patients. CONCLUSION: With long-term follow-up, It is possible to determine the effects of indirect hyperbilirubinemia and exchange transfusion. Effective screening of donor bloods will decrease the risk of post-transfusion hepatitis.
