Özet

Amaç:Preeklamptik eklamptik ve normal gebelerdeki serum Kolesterol, Trigliserid, HDL, LDL ve VLDL düzeylerini karşılaştırmak ve hastalığın şiddeti ile ilişkisini araştırmak. Gereç ve Yöntem:45 hafif, 36 ağır preeklamptik ve 50 normal gebe çalışma gruplarını oluşturdu. Gebelerin doğum öncesi ve postpartum 72. saatte serum Kolesterol, Trigliserid, HLDL, LDL ve VLDL düzeyleri kaydedildi. Buldular: Benzer demografik özelliklere sahip grupların doğum öncesi ortalama serum Kolesterol, Trigliserid, HDL, LDL ve VLDL değerleri sırasıyla hafif preeklamptik grupta 246,42 mg/dl, 251,0 mg/dl, 45,9 mg/dl, 134,6 mg/dl, 45,0 mg/dl, ağır preeklamptik grupta 251,15 mg/dl, 313,90 mg/dl, 51,11 mg/dl, 149,62 mg/dl, 92,0 mg/dl, 169,0 mg/dl, 167,36 mg/dl, 40,98 mg/dl, 85,18 mg/dl, 40,90 mg/dl olarak saptandı. Postpartum 72. saatteki değerler ise sırasıyla hafif preeklamptik grupta 238,04 mg/dl, 237,66 mg/dl, 44,33 mg/dl, 131,46 mg/dl , 40,66 mg/dl, ağır preeklamptik grupta 266,23 mg/dl, 273,18 mg/dl, 49,73 mg/dl, 138,50 mg/dl, 56,42 mg/dl, eklamptik grupta 216,16 mg/dl, 308,71 mg/dl, 43,0 mg/dl, 116,27 mg/dl, 83,57 mg/dl normotansif gebelerde 152,42 mg/dl, 151,58 mg/dl, 34,44 mg/dl, 82,32 mg/dl, 31,86 mg/dl, olarak saptandı. Preeklamptik ve eklamptik gruplarda hem doğum öncesi hem de doğum sonrası serum Kolesterol, Trigliserid, LDL-K değerleri normal gruba göre anlamlı olarak yüksek bulundu. HDL seviyeleri arasındaki farklar ise anlamlı değildi.VLDL-K değerleri ise sadece eklamptik grupta anlamlı olarak yüksek idi. Preeklamptik gruplar ile eklamptik grubun lipid düzeyleri birbirleri ile karşılaştırıldığında arada anlamlı farklar mevcut değildi. Sonuç: Preeklamptik ve eklamptik gebelerde HDL haricindeki serum lipid düzeyleri normal gebelerle kıyaslandığında daha yüksek bulunmuş olup, bunun preeklampsi patofizyolojisinde bir faktör olabileceğini düşünmekteyiz. Ancak hastalığın şiddeti ile korelasyon saptanmamıştır.

Abstract

Objective:To compare the serum cholesterol, triglycerides, HDL, LDL and VLDL levels of women with preeclampsia, eclampsia and normal pregnancies and to find out whether there is an association with the severity of the disease. Materıals and Methods:45 patients with mild, 36 patients with severe preeclampsia, 31 patients with eclampsia, 50 patients with normal pregnancies consisted the study population.The serum cholesterol, triglycerides, HDL, LDL and VLDL levels in the antepartumand 72 hours postpartum period were recorded. Results: The antepartum serum cholesterol, triglycerides, HDL, LDL and VLDL levels of the groups with similar demographic characteristics were 246,42 mg/dl, 251,0 mg/dl,45,9 mg/dl, 134,6 mg/dl, 45,0 mg/dl in mild preeclampsia, 251,15 mg/dl, 313,90 mg/dl, 51,11 mg/dl, 149,62 mg/dl, 92,0 mg/dl in severe preeclampsia 229,77 mg/dl, 381,27 mg/dl, 42,50 mg/dl, 126,62 mg/dl, 94,40 mg/dl in eclampsia and 169,0 mg/dl, 167,36 mg/dl, 40,98 mg/dl, 85,18 mg/dl, 40,90 mg/dl in normal pregnant women.The levels in the 72 hours Postpartum period were 238,04 mg/dl, 237,66 mg/dl, 44,33 mg/dl, 131,46 mg/dl, 40,66 mg/dl in mild preeclampsia, 266,23 mg/dl, 273,18 mg/dl, 49,73 mg/dl, 138,50 mg/dl, 56,42 mg/dl in severe preeclampsia, 216,16 mg/dl, 308,71 mg/dl, 43,0 mg/dl, 116,27 mg/dl, 83,57 mg/dl in eclampsia and 152,42 mg/dl, 151,58 mg/dl, 34,44 mg/dl 82,32 mg/dl, 31,86 mg/dl, in normal pregnant women.The antepartum and post partum levels of serum cholesterol, triglycerides and LDL levels were siginificantly higher in women with preeclampsia and eclampsia compared to normal pregnants.The HDL levels did not show significant differences. VLDL-K levels were only significantly higher in eclamptic women.The lipid content of women with mild, severe preeclampsia and eclampsia when compared with each other did not show significant differences. Conclusıon:Apart from HDL significantly higher serum lipid levels were found in preeclampsia and eclampsia compared to normal pregnant women, which may be a factor in the pathophysiology of preeclampsia.But it does not have an association with the severity of the disease.
