Pruritis vulvada kolposkopik biopsi bulgularının değerlendirilmesi

EÜTF Kadın Hastalıkları ve Doğum Anabilim Dalı 'na pruritis vulva şikayet ile müracaat eden ve kolposkopik gözlem altında alman biyopsi sonuçlarına göre vulvar nonneoplastik epitelyal bozukluk ve vulvar intraepitelyal neoplazi (VİN) tanısı alıp tedavi edilen 46 olgu prospektif olarak incelenmiştir. 18 olguda skuamöz hiperplazi (SN), 4 olguda liken skleroz (LS), 21 olguda skuamöz hiperplazi+liken skleroz ve 3 olguda (%6.5) VİN saptanmıştır. LS'lu olguların %75'inin 50yaş üzerinde, SH'li olguların %55.5'inin 50 yaş altında olduğu görülmüştür. VİN saptanan 3 olgudan 1 'ine skinning vulvektomi, 2 'sine basit vulvektomi operasyonu uygulanmıştır. 43 olguya lokal tıbbi tedavi verilmiş, 2 ay sonraki kontrollerde 26 olgunun semptomsuz olduğu, 13 olgunun semptomlarının azaldığı görülmüştür. Semptomları devam eden 4 olguya kolposkopik gözlem altında yeniden multipl vulvar biopsi yapılmış ve bu olgularda VİN saptanması üzerine 2 olguya skinning vulvektomi, 2 olguya basit vulvektomi uygulanmıştır. Pruritis vulva şikayeti ile müracaat eden olguların kolposkopik vulvar biopsi sonucuna göre lokal tıbbi tedavi büyük yarar sağlasa da, bu olguların takip edilmeleri, gerekirse yeniden kolposkop eşliğinde multipl vulvar biopsilerinin alınarak patoloji sonucuna göre tedavilerin yönlendirilmesinin yararlı olacağı sonucuna varılmıştır.
Abstract
Forty-six patients who underwent vulvar biopsies with the symptom ofpruritis vulva in Ege University Department of Gynecology and Obstetrics department were allocated to this study. Their pathologic findings were vulvar intraepithelial neoplasia and vulvar non neoplastic epithelial disorders and they were treated and controlled prospectively. Squamous hyperplasia (SH) were observed in 18 patients, lichen sclerosis (LS) in 4, squamous hyperlasia + lichen sclerosis in 21 and VIN in 3 patients. %75 of the LS patients were over the age of 50, %55.5 of the SH patients were below the age of 50. Skinning vulvectomy was performed on 1 and simple vulvectomy was performed on 2 of 3 VIN patients. Topical medication was applied to the other 43 cases and after 2 months follow-up, we observed complete healing in 26 cases and decreased symptoms in 13 patients. Symptoms continued in the remainder 4 of 43 patients and multipl vulvar biopsies were repeated under colposcopic examination on these cases. VIN were established at all of these 4 patients. We performed skinning vulvectomy on 2 and simple vulvectomy on the remainder 2 of these 4 patients. We concluded that local medication is effective for the patients complaining pruritis vulva but these cases should be followed-up and colposcopic repeated multiple vulvar biopsies should be done not to miss VIN cases.
