Özet

1995-1996 yıllan içinde BP-H nedeni ile medikal tedavi planlanan 41 hastanın tedavi öncesi semptom skor değerleri ile 6 haftalık doxazosin tedavisini takiben yapılan semptom skor değerlendirmelerinin prostatik volüm ve uroflowmetrik parametrelerle korelasyonu araştırıldı. Tedavi öncesi IPSS değerlerine göre oluşturulan ağır düzeydeki 21 hastanın ortalama 25,4 , orta düzeydeki 20 hastanın ortalama 16.1 olan IPSS puanları istatistiksel olarak anlamlı bir farklılık gösterirken (p 0.05). Buna karşılık tüm hastaların tedavi öncesi ve sonrası IPSS ve uroflowmetrik değerlerinin ortalamaları istatistiksel olarak anlamlı düzeyde iyileşme gösteriyordu (p<0.001). Tüm bu sonuçlar ışığında, semptom skor indexinin BPH'ın medikal tedavisinde takip kriteri olarak önemli bir parametre olduğu, fakat obstruksiyonun şiddetini daha objektif olarak belirleyen uroflowmetrik parametrelerle aynı ölçüde korele olmadığı, bu nedenle tedavinin güvenilirliği açısından hastaların çok yönlü değerlendirilmesinin başarı şansını artıracağı düşüncesine varıldı.

Abstract

41 benign prostatic hyperplasia (BPH) patients between 1995 and 1996 were planned medical treatment according to their symptom scors which were recalculated after 6 weeks of doxazasin treatment. These symptom scores before and after the treatment were compared with prostatic volume and uroflowmetric parameters. According to IPSS values before the treatment, 21 severe patient and 20 moderate patient had IPSS scors of 25.4 and 16.1 in average and, these difference was statistically significant (p<0.001). Although averages of Q max, residual volume- and prostatic weight of both groups of patient were close numbers, to together, they were not *'statistically significant. Similarly, although there was statistically significant difference between IPSS values 27 mild patients.( Average 12 ) after doxazosin treatment (p<0.001), this difference wasn't significant considering uroflowmetric parameters and average prostatic weights (p>0.05 ) between two groups of patinets. However, all natinets' IPSS and average uroflowmetric values before the treatment showed statistically significant improvement after doxazosin therapy (p<0.001). According to these results, we concluded that IPSS is an important follow up parameter for medical treatment of BPH but isn't correlated with uroflowmetric parameters which show severity of obstruction more objectively. So considering the reliability of the treatment, evaluation of patient with more than one parameter would increase the success rate.
