Neonatal klebsiella pneumoniae sepsisleri

Özet
Yenidoğan Yoğun Bakım Ünitesinde Klebsiella pneumoniae (n-43) ve diğer bakterilerle oluşan (n=19) sepsis atakları retrospektif olarak incelenmiştir. Erken ve geç sepsis dağılımı, başlangıç klinik ve laboratuvar bulguları, komplikasyonlar, yanıt ve tedavi süreleri ile mortalite oranları karşılaştınlmıştır. Nozokomiyal sepsis, Klebsiella grubunda daha sık gelişmiş (%86; %63, p<0.05) ve daha sık olarak (%41; %8, p<0.05) ilk 72 saatte başlamıştır. Başlangıç klinik bulgularından cilt renginde değişiklik (%65; %26, p<0.01) ve laboratuvar bulgularından trombositopeni (%72; %47, p<0.05) Klebsiella sepsislerinde daha sık saptanmıştır. Klebsiella grubunda, izlemde, solunum (%56; %37) ve dolaşım yetmezliği (%40; %16) ve anemi (%47; %21) daha sık görülmüş; bakteriyolojik yanıt süresi daha uzun bulunmuştur (10±4; 7+4 gün) (hepsi için p<0.05). Bu grupta, septik komplikasyonlara bağlı mortalite (%56; %22, p<0.01) de nozokomiyal sepsis mortalitesi de (%44; %21, p<0.05) daha yüksek bulunmuştur. Bu sonuçlarla giderek büyüyen bir hastane infeksiyonu sorunu olan ve yüksek mortalite taşıyan Klebsiella infeksiyonlarına karşı acil önlemler alınması gerektiği vurgulanmıştır.
Abstract
Septic attacks due to Klebsiella pneumoniae (n=43) and other bacteriae (n=19) were investigated retrospectively. Groups were compared in terms of early and late sepsis distribution, initial clinical and laboratory findings, complications, response to treatment, duration of treatment and mortality rates. Nosocomial sepsis was both more frequent (86% / 63%) and more frequently early-onset (41%/ 8%) in Klebsiella group (p<0.05 for both). Change in skin color (65% / 26%, p<0.01) and thrombocytopenia (72% / 47%, p<0.05) were significantly more frequent initial clinical and laboratory findings of sepsis in Klebsiella group. Respiratory (56%/37%) and circulatory failure (40%/16%) and anemia (47% / 21%) were encountered more frequently in Klebsiella group in the course of sepsis as well as later bacteriologic response to treatment (10±4 / 7±4 days) (p<0.05 for all). Septic mortality rate (56% / 22%, p<0.01) and mortality rate due to nosocomial sepsis (%44 / %21, p<0.05) were also higher in this group. It is concluded with these results that Klebsiella pneumoniae infections, a growing hospital infection problem with a high mortality rate necessitate urgent prevention measures.
