Özet

Demir eksikliği anemisinde tiroid bezi fonksiyonlarında ve tiroid hormonlarının soğuk stimulusuna cevabında bir azalma olduğu ileri sürülmektedir. Demir eksikliğinde hastalarda görülen ve direk olarak anemiye bağlanamayan semptom ve bulgularda, demir elementinin farklı sistemler üzerinde etkili olabileceğini düşündürmektedir. Bu çalışmada, demir eksikliği anemisi olan 62 hastanın demir replasmam öncesi ve sonrası tiroid hormon düzeyleri ölçülmüş; hemoglobin, hematokrit ve ferritindeki düzelmeyle tiroid fonksiyonları arasında ilişki kurulmaya çalışılmıştır. Demir eksikliği bulunan hastalarda demir replasmam ile düzelen ve ötiroid sınırlar içinde olan serum tiroid hormon düzeyi düşüklüğü saptanmıştır. Demir eksikliği anemisinde serum TSH, T3, ve FT3 değerleri için istatistiksel olarak çok belirgin bir düşüklük bulunurken, T4 için istatistisel anlamlı yükseklik tespit edilmiştir. FT4 düzeylerinde ise istatiksel olarak anlamlı fark bulunmamıştır. Serum TSH, T3, ve FT3 düzeyleri ile hemoglobin, hematokrit, serum 'demiri ve ferritin arasında pozitif korelasyon; demir bağlama kapasitesi ile negatif korelasyon olduğu saptanmıştır. Bu bulgular demir eksikliğinin tiroid bezi fonksiyonları üzerine olumsuz etkisinin varlığını, özellikle TSH, T3, ve FT3 değişikliklerinin belirgin olduğunu ve bu değişikliklerin demir replasmam ile giderilebildiğini ortaya koymaktadır.

Abstract

Iron deficient anemic patients have previously been claimed to have low plasma levels of thyroid hormone and a poor plasma thyroid hormone response to acute cold exposure. Symptoms and clinical signs, which are found in iron deficient patients, make us think iron deficiency may affect different systems by some mechanisms. In this study, thyroid hormone levels were determined before and after iron treatment in 62 patients with iron deficiency anemia. Plasma thyroid hormone concentrations were lower but all in euthyroid ranges in the iron deficiency state. These hormones rose to higher levels after iron replacement. After correction of hemoglobin, hematocrit and ferritin ; the statistical very significant rise in the plasma levels of TSH, T3 and FT3 were found. The statistically significant decrease of the plasma T4 level was observed. There was no statistically difference in the plasma levels of FT4 between iron deficient and iron replaced states. We found positive correlation between the plasma levels of thyroid hormones (TSH, T3, FT3 ) and some hematological parameters like hemoglobin, hematocrit and ferritin.This study revealed; iron deficiency has a negative effect on thyroid hormones. The thyroid hormone levels are lower in the patients with iron deficiency and the plasma levels of these hormones show an increase after correction of anemia.
