Özet
Sebebi açıklanamayan ikinci trimestir anne kanı hCG değeri yükselmesi ile gebeliğe bağlı hipertansiyon gelişimi arasında ilişki araştırılmıştır. 18-22 inci gebelik haftalarında 250 gebede hCG değerleri radioimmunoassay metodu ile ölçülmüş, hCG değerleri median 'in katları (MoM) olarak ifade edilmiştir. Maternal hCG değerinin ortalamasının gebeliğe bağlı hipertansiyon gelişen grupta [ (grup I, n=53), (32 597 mIU/ml=1.78 MoM) J, gebelikleri normal seyredenlere göre [ (grup II, n=197), (18 236 mIU/ml=l MoM) J daha yüksek olduğu saptanmıştır (p<0.01). Gebeliğe bağlı hipertansiyon gelişme insidansı grup I'deki gebelerde, grup H'ye göre istatistiksel olarak anlamlı derecede yüksek bulunurken (risk oranı: 3.25, %95 güven aralığı: 1.70-9.67, p<0.01), grup I'deki proteinürik gebeliğe bağlı hipertansiyon gelişme riskinin grup U'ye göre yaklaşık 5 kez arttığı saptanmıştır (risk oram:4.78, %95 güven aralığı: 1.96-15.71, p<0.01). Başka nedenlerle açıklanamayan ikinci trimestir maternal serum hCG yükselmesi gösteren gebelerin, ileriki gebelik haftalarında hem proteinürik hem de proteinüri olmadan gebeliğe bağlı hipertansiyon gelişimi açısından risk altında oldukları sonucuna varılmıştır.
Abstract
The correlation between the maternal serum hCG increase of unknown cause during the second trimester and the pregnancy, induced hypertension has been investigated. The hCG values have been determined by radioimmunoassey method in 250 pregnant women of 18-22 weeks of gestational age. The values have been stated as the multiples of the median (MoM). It has been found that the maternal hCG median values in the group of pregnancy induced hypertension [(group I, n=53), (32 597 mIU/ml = 1.78 MoM) ], were higher than the normotensive group [(group II, n=197), (18 236 mIU/ml = 1 MoM) ] (p<0.01). The incidence of pregnancy induced hypertension in the group I has been found to be higher than that in group II (risk ratio=3.25, %95 confidence interval=l. 70-9.67, p<0.01). In group I the risk of pregnancy induced hypertension with proteinüria has been found to be five times higher than that in group II (risk ratio=4.78, %95 confidence interval=1.96-15.71, p<0.01).It is concluded that pregnant women with high maternal hCG levels in second trimester of unknown cause are under the risk of pregnancy induced hypertension either with or without proteinüria.
