Amiloidozisli hastalarda böbrek ve duodenum biyopsilerinin karşılaştırılması

Özet

Amiloidozis tanısı biopsi ile konmaktadır ve son zamanlarda duodenal biopsi sonuçları,. amiloidozisli hastalarda %98-100 pozitif'sonuç■vermektedir. Bu nedenle biz bu çalışmada amiloidozisli hastalarda, renal ve duodenal biopsi sonuçlarını karşılaştırdık. Renal biopsileri pozitif olan 14 erkek, 10 kadın, toplam 24 hastada duodenum 2. kısmından endoskopik biopsi yapıldı. 19 hastada duodenal biopsiler yeniden pozitif bulunurken (%79.16), 5 hastada pozitif değildi (%20.84). Duodenum parafın blok doku kesitleri., sonuçları negatif olan, 5 hastada yeniden hazırlandı ve bu biopsi materyallerinin, Kristal Viole ile boyanarak amiloid depolanmasını gösterecek olan damar duvarı yapısını içermediği görüldü. Bu sonuçların negatif çıkması damar duvarı yapısının bulunmamasına ve Kristal Viole dışındaki tetkiklerin kullanılmamasına bağlandı. Bu nedenle negatif sonuçların teknik yetersizliğe bağlı olduğu düşünüldü. Bu ise bizim, literatürde beklenen daha yüksek sonuçlara ulaşmamızı etkileyen en önemli nedenlerden biriydi. 19 hastanın duodenal biopsileri metakromatik olarak boyandı ve pozitif kabul edildi (%79.16). Sonuç olarak bir hastada amiloidozisten şüphelenildiğinde, duodenal biopsi, daha riskli olan organlardan biopsi almadan önce denenebilir. Duodenal biopsi sonuçlarının, bizim çalışmamızda ve diğer serilerde pek çok diğer doku biopsilerinden daha güvenilir olduğu düşüncesine varıldı.

Abstract

The diagnosis of amyloidosis is established by biopsy and recently the duodenal biopsy results are positive 98-100 percent in the patients with amyloidosis. Because of this reason, in this study we compared the renal and the duodenal biopsies results in the patients, with amyloidosis. 24 patients (14 men, 10 women) whose renal biopsies were positive for amyloidosis are rebiopsied from the second part of the duodenum, endoscopically. 19 patients'duodenal biopsies were positive again (79.16%), 5 were not (20, 84%). The paraffin block tissue sections of duodenum are repreperad in the negative 5 patients, and it has become clear that these biopsy materials did not contain the vessel wall to show the perivascular amyloid deposites, with Cristal Violet. So that the negative results are thought to be due to use only Cristal Violet stain not the others and technical insufficiency. The 19 patients' duodenal biopsies were stained metacromatically and positive (79.16%). As a result, We think that, when the diagnosis of amyloidosis is suspected in a patient, the duodeal biopsy can be performed previously, instead .öf the other organs those are more risky and the results are more reliable than most of the other tissue biopsies, in our and others'series.
