Truncus lumbosacralis'in anatomik varyasyonları: Anatomik kadavra çalışması

Özet
AMAÇ: Medulla spinalis'in lumbal 4. ve 5. segmentlerinden çıkarak plexus sacralis'in yapısına katılan sinirlere tr. lumbosacralis denilmektedir, instabil pelvis kırıklarında art. sacroiliaca'nın ön taraftan tesbitinin gerektiği durumlarda tr. lumbosacralis'in buradaki seyri önem kazanmaktadır. YÖNTEM: Diseksiyon 18 kadavra üzerinde yapılmıştır. Diseksiyon sırasında Opmi l diseksiyon mikroskobu kullanılmıştır. BULGULAR: Klasik tr. lumbosacralis'e 12 kadavrada rastlanmıştır. Geriye kalan 4 tanesinde L4 ve L5 sinirleri linea terminalis'in ortalama 23 mm altında tr. lumbosacralis'i oluştururken, 2 tanesinde tr. lumbosacralis 'in oluşmadığı tesbit edildi.SONUÇ: Buna göre art. sacroiliaca 'ya önden yapılacak girişimlerde L4-L5 köklerinin farklı seyirlerinin bilinmesi ameliyat sonrası komplikasyon riskini azaltacaktır.
Abstract
PURPOSE: Lumbosacral trunk is formed by the L4 and L5 segments of the spinal cord. When performing procedures on the anterior side of the sacroiliac joint the exact location of the plexus gains importance. METHODS: We have dissected 18 cadavers and used Opmi l disection microscope. RESULTS: Only 12 of them had the classic truncus. In the other 4 cases, the L4 and L5 nerve roots formed the lumbosacral trunk an average of 23 mm below the terminal line. In 2 cases the two nerves did not form a truncus. CONCLUSION: A true knowledge of the nerve roots is essential in performing surgeries for the anterior sacroiliac joint.
