Özet

AMAÇ: Demir eksikliği anemisi (DEA) olan çocuklarda oral ve parenteral (intramüsküler, i.m.) demir tedavisinin etkinlik, yan etki ve hasta uyumu yönünden karşılaştırılması. GEREÇ VE YÖNTEM: Hastalar, uygulanacak tedavi yöntemine göre oral (ODTG, n=30) ve parenteral PDTG, n=22) demir tedavi grupları olarak ikiye ayrıldı. Çalışma 40 hasta ile tamamlandı (her iki grupta 20 'şer hasta). PDTG, oral demir tedavisinin uzun zamanlı oluşunu (en az üç ay) gerekçe göstererek kabul etmeyen annelerin çocuklarından oluşturuldu. Hastaların tam kan ve serum demir değerleri 0., 1. ve 3. aylarda olmak üzere üç kez; sağlıklı çocukların ise bir kez çalışıldı. Tedavi olarak; ODTG 'ndaki hastalara oral demir 2+ (Ferrous sulfate) 6/mg/kg/gün (3 ay) ve PDTG 'ndakilere ise total demir gereksinimlerine göre intramüsküler demir (Ferric III hydroxide polymaltose, 8-10 gün) verildi. BULGULAR: ODTG'undaki hastaların %33.3 (10/30)'nde tedaviye uyumsuzluk ve %26.7 (8/30)'nde de yan etkiler görülürken, PDTG 'ndaki hastaların daha düşük bir kısmında (%9.1, 2/22) uyumsuzluk ve yan etki (% 13.6, 3/22) görüldü. Kontrol, ODT ve PDT gruplarının (başlangıç, 1. ve 3. aylardaki) Hb (ortalama±SD, g/dL) ve ferritin (ortalama±SD, ng/mL) değerleri sırası ile şöyle bulundu: kontrol Hb /ferritin: 13.1±0.62 / 66.1±29.5; ODTG Hb /ferritin: 7.9±l.l / 2.6±2.4, 11.2±1.2 / 20.3±14.5, 13.3±0.5 / 63.1±16.8; PDTG Hb: 7.5±1.1 / 3.8±3.1, 10.7±0.9 / 102.7±32.1, 12.9±1 / 42.3±22.5. SONUÇ: Bu sonuçlara göre, DEA 'nin tedavisinde ODT ve PDT eşit etkilidirler; ancak PDT'nin ODT'ne göre tedavi süresinin kısalığı, yan etki ve hasta uyumu yönünden üstünlükleri vardır.

Abstract

OBJECTIVE: In children with iron deficiency anemia (IDA), the comparison of oral and parenteral (intramuscular, i.m.) iron supplementation therapies in terms of efficacy, side effects and patient compatibility. MATERIAL AND METHODS: The patients were divided into two groups according to the route of administration oral (oral iron therapy group, OITG, n=30) and parenteral groups (parenteral iron therapy group, PITG=22). The study was completed on 40 patients (each group contained 20 patients). PITG consisted of the patients whose parents refused to apply oral iron supplementation therapy because of its long duration. In the patients, whole blood and serum iron values were studied on the months 0, I and 3, whereas those of the healthy children were examined once only. The patients in the groups of OIT and PIT received oral Fe 2+ (ferrous sulphate, 6 mg/kg/day for three months) and i.m. iron (Ferric III hydroxide polymaltose, depending on the degree of deficiency, the duration of 8-10 days), respectively. RESULTS: In the OITG, a rate of discompatibility (n=10/30, 33.3%) and side effects (8/30, 26.7%) were encountered, whereas a lower rate of discompatibility (n=2/22, 9.1%) and side effects (3/22, 13.6%) were seen in the PITG. In the patients and healthy children, the values of Hb (mean±SD, g/dL) and ferritin (mean±SD, ng/mL) on the months 0, 1 and 3 were found as follows, respectively: the control Hb / ferritin: 13.1±0.62 / 66.1±29.5; OITG Hb /ferritin: 7.9±1.1 / 2.6±2.4, 11.2±1.2 / 20.3±14.5, 13.6±0.5 / 63.1±16.8; PITG Hb / ferritin: 7.5±1.1 / 3.8±3.1, 10.7±0.9 / 102.7±32.1, 12.9±1 / 42.3±22.5. CONCLUSION: These results indicate that OIT and PIT are equally effective in the treatment of iron deficiency anemia, but PIT is more useful than OIT in terms of its short duration, side effects and patient compatibility.
