Rüptüre ve semptomatik abdominal aort anevrizmalarının tedavi sonuçlarının karşılaştırılması (Retrospektif klinik araştırma)

Özet
AMAÇ: Bu çalışmada Ankara Üniversitesi Tıp Fakültesi Genel Cerrahi Ana Bilim Dalında Ağustos 1998-Nisan 2001 tarihleri arasında ameliyat edilen semptomatik ve rüptüre Abdominal Aort Anevrizmalarının(AAA) tedavi ve takip sonuçlarının karşılaştırılması amaçlanmıştır. GEREÇ VE YÖNTEM: Yaş ortalaması 75.4 olan 28 (3 kadın) semptomatik ve yaş ortalaması 67.1 olan11 (1 kadın) rüptüre AAA hastası tedavi ve takip edilmiştir. Hastalar anevrizmanın yerleşimi, çapı, türü, yapılan ameliyat, kullanılan materyal, sistemik ve girişime özgü komplikasyonlar, tekrarlanan girişim ihtiyacı, morbidite ve mortalite, hastanede kalış süresi açısından değerlendirilmiştir. BULGULAR: AAA yerleşimi semptomatik grupta bir supra-renal ve rüptür grubunda bir juxta-renal dışında infrarenaldir . Tüm anevrizmalar fusiform tiptedir. Semptomatik grupta 15 hastada aorta bi iliak,5 hastada aorta bi femoral, 8 hastada tüp greft, rüptür grubunda 6 hastada aorta bi iliak,1 hastada aorta bi femoral,4 hastada tüp greft kullanılmıştır. AAA'larının çapı karşılaştırıldığında semptomatik grupta çap ortalama 7.1 iken rüptür grubunda 8.6 olup fark istatistik olarak anlamlı bulunmuştur. (p<0.05) Ortalama izlem ve ortalama hastanede kalış süresi semptomatik grupta sırasıyla 16.0 ay ve 14.8 gün, rüptür grubunda ise 15.6 ay ve 18.0 gün olmuştur ve arada anlamlı fark yoktur. Semptomatik grupta 3, rüptür grubunda 6 hasta hastanede yatarken exitus olmuştur. (p<0.01) SONUÇ: AAA rüptürünün yüksek mortalite ve morbidite ile seyreden bir hastalık olduğu ve AAA sında erken tanı ve tedavi ile yüz güldürücü sonuçlar elde edilebileceği göz önüne alınarak ciddi sırt ve karın ağrısı olan ileri yaş hasta grubunda AAA akla gelmelidir.
Abstract
AIM: The aim of this study is to compare the resuts of the management of symptomatic and ruptured abdominal aortic aneurysms (AAA) operated between August 1998-April 2001 in the surgery department of Ankara University Medical School. MATERİAL AND METHODS: Symptomatic 28 patients(3 female) mean age 75.4 and ruptured 11 patients of AAA (1 female) mean age 67.1 were operated. Patients were evaluated for the placement,diameter, and type of the aneurysm,type of the operation, operative material, systemic and operative complications, reoperation, hospital stay, morbidity and mortality. RESULTS: Placement of all the aneurysms except a supra-renal in the symptomatic group and a juxta-renal in rupture group, was infra-renal. All AAAs were fusiform. Fifteen aortobiiliac, 5 aorto bifemoral and 8 tube greft in the symptomatic group and 6 aortobiiliac, 1 aorto bifemoral and 4 tube greft in the rupture group were used. Mean diameter of the AAA in the symptomatic group was 7.1 and 8.6 in the rupture group.(p< 0.05) Mean follow up and hospital stay period were 16.0 month and 14.8 days in the symptomatic group and 15.6 month and 18.0 days in the rupture group respectively. Operative mortality was 3 in symptomatic group and 6 in the rupture group. (p< 0.01) CONCLUSION: Since AAA rupture is a potentially lethal condition and early diagnosis and treatment can be life saving it should always be kept in the mind especially in the patients with severe low back or abdominal pain.
