Postmenopozal osteoporozlu hastalarda levotiroksin-sodyum tedavisinin kemik mineral yoğunluğuna etkisi

Özet
AMAÇ: Levotiroksin tedavisinin kemik mineral yoğunluğuna etkisi halen tartışmalıdır. Osteoporotik postmenopozal hastalarda levotiroksin kullanımı hususu da netlik kazanmamuştır. Bu retrospektif çalışmada amacımız levotiroksin tedavisinin kemik mineral yoğunluğuna etkisini ve osteoporoz tedavisinin sonuçlarını etkileyip etkilemediğini araştırmaktı. GEREÇ VE YÖNTEM: Çalışmaya halen levotiroksin tedavisi almakta olan 26 postmenopozal osteoporotik hasta ve yaşları uyumlu, levotiroksin kullanmayan 146 postmenopozal osteoporotik kontrol hasta alındı. Osteoporoza yönelik tedavi başlanmadan önceki veriler bir yıllık tedavi sonrası elde edilen verilerle karşılaştırıldı. BULGULAR: Levotiroksin kullanan hastalar ile kontrol grubu yaş (sırasıyla 59.2±6.3'e karşı 59.9±8.4 yıl; p>0.05), menopoz yaşı (sırasıyla 44.6±5.9'a karşı 46.8±6.2 yıl; p>0.05), menopoz süresi (sırasıyla 14.5±10.2'e karşı 14.3±7.3 yıl; p>0.05), ve tedavi öncesi kemik mineral yoğunluğu (KMY) (sırasıyla 0.780±0.080'e karşı 0.770±0.110 g/cm2; p>0.05) yönünden benzer bulundu. Tüm hastalara bifosfonat ve kalsiyum tedavisi verilmişti. Tedavi ile levotiroksin grubunda KMY %7.3 (0.780±0.080'den 0.840±0.160 g/cm2'e; p<0.001), kontrol grubunda ise %5.8 (0.770±0.110'den 0.810±0.100 g/cm2'e; p<0.001) artış gözlendi. İki grup arasında KMY'daki değişim (delta BMD) farklı değildi (p<0.05). SONUÇ: Sonuçlarımız postmenopozal hastalarda levotiroksin tedavisinin ne KMY düzeyini ne de bifosfonataların terapötik etkinliğini etkilemediğini göstermektedir.
Abstract
AIM: The effect of levothyroxine treatment on bone mineral density is still controversial. There is also a controversy in using levothyroxine in osteoporotic postmenopausal patients. In this retrospective study we aimed to investigate the effect of levothyroxine treatment on bone mineral density. MATERIALS AND METHODS: Twenty-six postmenopausal osteoporotic patients who were currently on levothyroxine treatment, and 146 age-matched control patients are involved in the study. Data recorded before the initiation of treatment for osteoporosis are compared with the data obtained after one year of treatment. RESULTS: Patients on levothyroxine treatment were similar to the control group with regard to age (59.2±6.3 vs. 59.9±8.4 years, respectively; p>0.05), age at menopause (44.6±5.9 vs. 46.8±6.2 years, respectively; p>0.05), duration of menopause (14.5±10.2 vs. 14.3±7.3 years, respectively; p>0.05), and pretreatment bone mineral density (BMD) (0.780±0.080 vs. 0.770±0.110 g/cm2, respectively; p>0.05). All of the patients were given biphosphonate plus calcium treatment. Treatment resulted in 7.3% increase in BMD (delta BMD) in the levothyroxine group (from 0.780±0.080 to 0.840±0.160 g/cm2; p<0.001), and 5.8% increase in the control group (from 0.770±0.110 to 0.810±0.100 g/cm2; p<0.001). The D BMD was not different between the two groups (p<0.05). CONCLUSION: Our results indicate that levothyroxine treatment neither affects BMD levels nor interferes with the therapeutic efficacy of the biphosphonates in postmenopausal patients.
