Özet

Son çalışmalar kitlesel hepatit B aşılamasının hepatit B virüs(HBV) enfeksiyonunundan korunmada ve HBV'nin endemik olduğu ülkelerde çocuklarda primer hepatosellüler karsinoma gelişmesini önlemede etkin olduğunu göstermektedir. Günümüzde 110'dan fazla ülkede bir yaşından küçük çocuklara universal HBV aşılaması uygulanmaktadır, ancak daha büyük çocukların ve adolesanların aşılanması birçok ülkede istenen düzeyde değildir. Bu çalışmada ilimizde yer alan dört farklı ortaöğretim okulunda yatılı olarak öğrenim görmekte olan yaşları 14-17 arasında değişen toplam 360 sağlıklı öğrenciden serum örnekleri alınarak mikro EIA yöntemiyle HBV ile ilgili serolojik göstergeler (HBsAg, antiHBcIgG, antiHBs) araştırılmıştır. Toplam HBV seroprevalansı bu yaş grubunda % 10.8 olarak saptanırken, 8 olgu (% 2.2) HBsAg pozitif, 24 olgu (%6.6) bağışık, 316 olgu (%87.8) duyarlı, 7 olgu (%2) tek başına antiHBcIgG pozitif, 5 olgu ise (%1.4) aşılı olarak saptanmıştır. Bu sonuçlar bölgemizde ve ülkemizde bir yaş altı çocuklara yapılan universal HBV aşılamasının yanısıra 1-18 yaş arasında yer alan çocukların da kitlesel aşılama programına dahil edilmesinin gerekli olduğunu göstermektedir.

Abstract

Recent studies conclude that mass hepatitis B immunization is effective in preventing Hepatitis B virus (HBV) infection and has resulted in a decrease in the occurrence of primer hepatocellular carsinoma in children living in countries where hepatitis B is endemic. An extended program of immunization including hepatitis B vaccine for children under one year of age has been launched in more than 110 countries, but the older children and adolescents' immunization has not been launched enough in many countries. In this study, blood specimens were collected from 360 healthy adolescents aged between 14-17 years for determination of HBV seromarkers (HBsAg, antiHBc IgG, anti HBs) by enzyme-linked immunosorbent assay commercial kits. The results showed that the seroprevalence of HBV seromarkers was 10.8 %, the HBsAg carrier rate was 2.2 %, the anti HBs and antiHBcIgG positivity rate was 6.6%, the prevalence of only antiHBc IgG was 2 % and the prevalence of only antiHBs positivity was 1.4%. HBsAg prevalence rate and also vaccination rate in adolescents of the present study was low and rate of sensitive adolescents to HBV infection was found high. Therefore, HBV vaccination schedule should be expanded from 1 year to 18 years in addition to universal ımmunization of infants in our region and our country.
